How about this amazing study for a post!

A 3-month old out of hospital cardiac arrest occurred in UK, with a presenting rhythm of V-Fib. A total of 10 defibrillations were performed over a 25 minute cardiac arrest using and AED with adult pads. ROSC was finally achieved on scene (they didn't rush off scene 👍).

What are the notable aspects of this case?
1. The patient had recurrent VF as the presenting rhythm(typically less than 10% of all infant arrests) and received 10 appropriate AED shocks.
2. The AED effectively terminated the rhythm yet it continued to recur. This is not 'refractory' Vfib, instead it's recurrent Vfib, meaning that dual sequential defibrillation (DSED) would not have worked here.
3. The stay on scene strategy was life-saving.
4. ROSC occurred 7 minutes after the last epi possibly implying that clearing it from the system actually helped establish a perfusing rhythm (i.e. Epi may be harmful in VF).
5. The early resumption of compressions without rhythm analysis was associated with re-fibrillation from an organized rhythm showing that we do miss ROSC sometimes and likely reinduce VF even as we monitor EtC02 and look for other signs of native perfusion.

It’s not everyday you get to work with a “true mentor”. I often complain that we use the term “mentor” to loosely. There’s also not many people I call “mentor”.
Ian “Tucks” Tucker is one of them .

I first met Tucks in December 2006 when my family and I traveled to Australia to investigate a possible sea change. I did a couple ride alongs with Tucks on the Gold Coast quick response unit and afterwards he gave me my first intensive care paramedic epaulets and gold caduceus. I still have them to this day.

When I was training to become an ICP Tucks was one of my mentors. It was the compassion and friendship that he bestowed on me, our colleagues and patients that I’ve tried to emulate throughout my career. Qualities not taught in a book or classroom but past down by true leaders, good humans and quality mentors.

Tucks it was a privilege to have met and be trained by you. Days before your retirement I am overjoyed to have competed this case with you.

Semper Fi and god speed. I pray your final working days are filled with colleagues like me making sure you know what an influence you had on them.

Flumazenil (Romazicon) is a reversal agent for benzodiazepines ("benzos") through competitive inhibition at the GABA(A)/benzo receptor complex. Since it serves as an antidote for benzos, we have to consider the effects of acute withdrawal - anxiety, agitation, and even lowering the threshold for seizures. Similar to naloxone for opioid reversal, flumazenil’s relatively short half-life may require additional doses to mitigate the effects of long-acting benzos. 💉😱
The most common reason I administer flumazenil is to reverse sedation and respiratory depression by giving 100-200 mcg intravenously every minute (max of 3 mg in an hour) while ruling out other potential etiologies of the symptomatology at hand. Because I don’t utilize benzos that often in my practice, I’ve only had to use it a handful of times in my career. 👨🏽‍⚕️
Since ethanol and benzodiazepines both work on the GABA(A) receptor, does flumazenil help with reversing the central nervous system effects of alcohol toxicity? Some of the evidence suggests no benefit, and other studies suggest at higher doses that it may indeed help. From a pharmacokinetic standpoint, benzos and alcohol work at different allosteric sites on the receptor, and in general, alcohol withdrawal is treated with BENZOS (not benzo REVERSAL). I prefer a phenobarbital taper. 🤙🏽😷
Here's one for the chemistry majors - the fusion of a BENZENE ring and DIAZEPINE ring forms the core molecular structure for benzodiazepines (hence the name). 🤓

Of note, while they do live on a sail boat, in some of the most pristine waters known to man, Ryan fights a disease that is ravaging his body. All the while he smiles, says kinds words and attacks life with the gusto of a fighting Marine at Bellea Wood (where the Germans nicknamed us Devil Dogs).
