I would like to share this with other families who I know have been struggling for a long time to help their children with toilet training issues. Not only do I help children with normal development, but also those who have a variety of other delays.

This was written by a parent of a 12 year old girl with a chromosomal abnormality that meant she was non-verbal, and had intellectual and other developmental delays. She has been one of my biggest challenges in recent years but also one of the most rewarding- simply because her parents never gave up!

"I think we when first saw you, we had a good gut feel that XX was ready, she just needed the tough love nudges that we worked on with your guidance, direction and support. I know as the parents you need to be 100% committed, weather the high and lows of the toileting journey as you said to her dad and I time and again, and “it will be messy” and it was.

Statistically speaking we started our toileting journey with you on Thursday, 9 May 2024 and roughly 3 to 4 months later, XX was wee’ing in the toilet (with a few accidents), by Sept / Oct 2024 XX was dry overnight with a few incidents and at the beginning of 2025 to mid way through 2025, I think XX was 95% toilet trained.

As you would recall during the last legs of XX journey, that is her no2s I was about to give up and throw in the towel, I had almost given up until she did her no2s in the toilet during her bath. The progress and steps of her no2s was slow and sometimes arduous but we hung in there, sometimes by our finger nails (lol).

It’s always important as the parent to not lose hope no matter the odds or how arduous the journey is proving to be, we need to be diligent, vigilant, tenacious and above all forgiving when things don’t go as we hope or expect them to.

XX is also going to various toilets without issue now because “a toilet is toilet” regardless of where and “when you gotta go, you’ve gotta go”

When a child is ready to be toilet trained, they will often not tell a parent of their need to poo or wee. Instead they will “hold on” or “hide”. This clearly demonstrates they are aware before they poo/wee, but then often refuse to go to the toilet when prompted.

The biggest mistake parents make, is that they ask their child “do you need to go to the toilet?” or “do you need to do a wee/poo?”. This give the child the option to say “no” and of course if parents then insist they go to the toilet, this inevitably results in a meltdown/argument and wet/pants or soiling shortly afterwards.

At the moment it is my busy time of the year. Many of the children I see are getting ready to go to school next year, and are often still holding on and waiting for a pullup, or are soiling or wetting themselves. Now is the time to seek help if your child is still struggling with these behaviours.

I am becoming increasingly concerned that children are suffering more these days from not being offered privacy for doing their “ones and twos” at daycare facilities, kindergartens, and preschools. Typically, most toilets are designed with no doors and offer open cubicles for children who are learning how to use the toilet for the first time.

As a result I continue to see hundreds of children each year who become constipated, withhold both wees/poos for many hours, wet and/or soil themselves because they do not feel comfortable or safe in using them.

It a basic human instinct to seek privacy for doing “Ones and Twos”, which is not being offered at daycare facilities for young children in Australia.

As parents increase the time they spend at work (either by choice or necessity), the children have less time to be toilet trained in the calm environment and privacy of their home. So children are becoming increasingly stressed and anxious, and suffer the ultimate humiliation and indignity of soiling or wetting themselves constantly at home or at daycare. This in turn causes more stress for their parents and educators.

At this time of the year, there are thousands of children getting ready to start school next year. And so many of these children still lack the ability to stay dry and clean all day because they are not yet fully toilet trained.

Unfortunately this puts a financial strain on the families, and I am seeing an increasing number of children who require NDIS funding, or make claims with Medicare, to help fix this problem.

Wouldn’t it be easier to just put a few doors on toilet cubicles in all childcare facilities instead?

Good news for those people who need my support for their "Ones and Twos" problems, especially as the preschoolers are getting ready to go to school next year. I will be extending my hours from 8am- 5pm from Monday-Thursday and am now available from 8am- 3.30pm Friday as well.

Please be aware that you can ask your GP for a care plan which will enable you to make a claim for a partial refund with Medicare. I am also able to see clients with self managed NDIS funding, and will consider all plan managed clients also.

I have been called a number of funny names during my career in the world of "Ones and Twos" - poo doctor, poo lady, poo queen etc. I recently listened to a podcast about another inspirational Australian who isn't afraid to talk about toileting issues and has the original title of "toilet warrior". This story is definitely worthwhile listening to!

So often I hear that parents think it is ok to let their children sit on the toilet with an iPad/screen to encourage them to poo.

Regardless of whether it is during toilet training, or as a result of constipation and soiling, it is something I strongly advise against. Screen time is much better used as a reward for passing poos in the toilet, or additionally having a clean day as well.

I have plenty of other suggestions to help manage children's opposition, anxiety, and reluctance to sit on the toilet. Please get in touch and book an appointment with me if you have any behavioural toileting issues that can result in encopresis, functional constipation, and soiling.

Well it is nearly the end of another busy school term, and after seeing many clients (over 1200 sessions this year), I am about to go on a short two week break over the Easter holidays. New clients can book online to see me when I return on 17 April and I will be finishing on 30 March.

Thank you all for your wonderful support throughout the year. I feel so privileged to have been able to help so many families again with their Ones and Twos problems.

I will be away from 9/12/22 until 16/1/23 for a holiday as well.

Bookings can still be made online via the website but I am currently booked out until February 2023.

Please be aware that you will need to complete both the clinical and payment forms before you will be eligible for therapy in 2023.

Make sure you also book an initial (60 minute) consultation followed by 5 standard (30 minute) consultations to ensure you get the best possible options for you and your child.

Ones and Twos Occupational Therapy has been developed by Christine Shearer-Smith to help children who have toileting difficulties. They often struggle with toilet training, constipation, soiling or wetting during the day, or bedwetting at night.
