As many participants plans reduce and funding becomes an issue here it a vital form to help with discussions with your NDIS or Lac coordinator and support coordinators when it’s time for a review. Or for those new to a plan. Please note: not all Social Workers do every one of these roles.
Some work as a Support Coordinators, some as FCA (functional capacity assessments) and others doing assessment and therapy under Capacity Building and other roles eg: employment assistance etc. Some work in a combination of differing skills across the NDIS.
