We offer a simple and accessible referral program at Hands On People.

You can submit a referral in just a few minutes
or provide more details if needed.

Once received, our team carefully reviews the information
and reaches out to understand the participant’s needs.

Referrals can be made by participants, families,
or coordinators with consent.

A lot of plans include Community Access.
But in reality, not everything in the plan turns into actual support.

Appointments get missed.
Outings get cancelled.
Support changes too often.

We focus on making the basics work properly.

Getting someone to where they need to be, on time.
Keeping routines consistent.
Making sure transport is organised and accessible.
Having support workers who show up and know the person.

That’s what makes the difference day to day.

Not all support is effective even when it appears stable.

In practice, we often see situations where:

• routines are followed
• reports are completed
• there are no incidents

but there is little or no measurable progress.

This is where the distinction between task-based support and outcome-based support becomes critical.

Under the NDIS framework, support is expected to build capacity, improve participation, and lead to meaningful outcomes over time.

If those outcomes are not visible, it may be a sign that the support model needs to be reviewed.

For coordinators and families, this means looking beyond activity and asking:
