..but let's talk about the End-of-Life Pathway in the Support at Home program

This is a new funding pathway that did not exist under the Home Care Package program

Older adults diagnosed with less than 3 months to live have access to $25,000.00 in funding which runs in parallel to any palliative services delivered by states and territories

Like all aged care programs, access is through My Aged Care

At CT, our allied health services are here to support people during palliative care with Speech Pathology, Dietetics and Occupational Therapy often involved

Okay...let's talk Support at Home Quarterly Budgets...
..so much is new in the Support at Home program
..and there are new rules about what happens with "unspent funds" in each quarter

It is now really important that participants and care partners work together to maximise the use of their funding each quarter to meet their care needs and goals 😊

Our allied health services have no contributions...
..and that includes any work we do to support equipment and home modification prescriptions

Support at Home - can you use only your AT or HM budget? πŸ€”
..the short answer is yes!

This was an unexpected part of the reform to be honest

And the claiming rules for AT-HM makes it difficult for registered providers to only deliver these and meet all the responsibilities under the Aged Care Act sustainably

The key takeaway here is that if you are only looking to engage in your Assistive Technology or Home Modifications budget, and aren't going to take up your Support at Home classification....
..you may find that not all providers will offer this service
..and you need to call around

As an associated provider, our allied health professionals at Community Therapy can provide the required assessments, reports, equipment trials, home modifications, and plans...
..however, we need a referral from a registered provider to support you 😊

What is the Restorative Care Program? πŸ€”

This is a 16 week program to support older adults to increase their functional capacity and independence to live longer and better at home

It is accessed via My Aged Care and an aged care assessment

It replaced a previous program called the Short Term Restorative Care program

What do you do?

Well, in your decision letter it will include information about the pathway to appeal

The first step is writing to the Secretary of the Australian Government, Department of Health, Disability and Ageing, outlining why you think the decision should be changed. You have 28 days to do this after receiving your Notice of Decision letter

Following this, if you're still not happy with the outcome, you're able to go through to the Administrative Review Tribunal, but there will likely be a fee for this. You can learn more about the tribunal here - https://www.art.gov.au/
