Your Community Health (previously known as Darebin Community Health) is a health and wellbeing service provider. We offer free and affordable services to anyone in northern Melbourne. We provide services at our three health centres:

East Reservoir Health Centre, 125 Blake Street, East Reservoir, VIC 3073

Northcote Health Centre, 42 Separation Street, Northcote, VIC 3070

PANCH (Preston) Health Centre, 300 Bell Street, Preston, VIC 3072

We also provide some services are available in your home or in other community settings, such as Reservoir Leisure Centre. Our services include:

This Apology acknowledges the deep harm caused to Aboriginal communities across generations and marks an important step in Victoria’s journey toward truth, justice and Treaty.

As 16 Days of Activism continues, we are proud to advocate, raise awareness, and amplify voices. Today, a member of our community has chosen to share their experience and reflect on the generational impacts of family violence and their journey toward healing.

"When you silence yourself again and again—shrinking your needs, minimising your emotions—your once-strong posture, built on generational love, crumbles like a weathered shack. But inside, something begins to grow: a voice. From the pain, loss, and heartbreak, that whisper swells into a fierce, generational scream.

Healing is personal. I lived in silence, shadowed by shame, hanging on with trembling fingers. Change arrives only when the desire to move on outweighs the desire to hold on. So now, I quiet my mind, take a deep breath, and leap into the unknown—because I refuse to watch another generation suffer trauma, anxiety, addiction, and loss.

I endured miscarriages, spinal surgeries, depression, su***de attempts, homelessness. Yet amidst despair, hope transformed into confident expectation—esperance—that this journey is bigger than me. Family violence in Australia feeds this cycle of pain, tearing communities apart, where conditional love breeds wounds passed across generations.

Kids learn to earn love through silence and perfection, carrying this ache into adulthood. But connection, and unconditional love break the cycle. By choosing presence over perfection, we begin to heal the cracks in our souls—and maybe, just maybe, in our world too."

Meet Sandaleen! One of the passionate volunteers featured in our Annual Report.

After moving to Darebin, she wanted to give back and feel more connected to her new community. Joining our Community Advisory Committee gave her the chance to do just that.

From day one, Sandaleen has felt valued and inspired by the people around her. She says one of the best parts of volunteering has been meeting others who care deeply about creating safe, supportive spaces where every voice is heard.

Today we mark World AIDS Day, an international day first held in 1988 to raise awareness about HIV and AIDS, remember those who have died from AIDS-related illnesses, and show support for people living with HIV.

In Australia, an estimated 30,000 people are living with HIV, and worldwide, 39.9 million people are living with the virus, with more than 42.3 million lives lost to AIDS-related illness. World AIDS Day is a chance to raise awareness, eliminate stigma, and promote prevention, treatment, and care.

This year’s theme is 'No one left behind', highlighting the importance of inclusion, understanding, and support for everyone affected by HIV.

We are honoured to have won the ‘Creating a Sustainable and Climate Resilient Health System’ award at last night's Victorian Public Healthcare Awards!

Our Climate Action work brings together staff, clients and community members to develop effective, practical solutions including fleet electrification, rooftop solar and better waste management, to improve our sustainability. These actions have cut our emissions by 33% and driven meaningful change across the organisation.
