Ever wondered what the ‘pop’ was during a joint manipulation or adjustment?

It is not your actual bones, it is little gas bubbles being formed and released into the joint space as a result of the pressure changes when a joint is placed under maximum bind/compression. This reduces the pressure within the joint space and improves range of movement.

HVLA (high velocity, low amplitude) techniques are thought to provide a strong stimulus to the neurological system in order to reset abnormal signalling, relax overactive muscles, reduce pain and restore joint range of movement.

This technique is just one of many that your osteopath may use and may not be suitable for everybody.

Chronically tight adductor (inner thigh) muscles contribute to a lot of sacroiliac joint and hip joint pain as well as p***c symphysis issues and pelvic floor tightness. They can also the key to improving pelvic floor, core and hip function.

Adductors Magnus, has 2 portions of the muscle with one portion flexing and internally rotating the thigh, whilst the other extends and externally rotates the thigh. Both portions act to adduct (bring closer to the midline) the femur.

Depending on which attachment point is fixed/stabilised will depend on the action of the muscle. For example, if the femur is fixed, the adductor moves the pelvis towards the femur allowing for a widening of the pelvic outlet. This in turn allows for a lengthening through the posterior hip and pelvic floor musculature, a shortening of the deeper core transverse abdominus and a lifting of the pelvic floor sling.

Dominant and overachieving adductors also inhibit the ability of the glutes to show up and activate to stabilise the pelvis, which can result in altered mechanics and loading patterns through the lumbar spine.

There is so much more potential when appropriate strengthening of the adductors is included to unlock the chronic low back, pelvic and hip dysfunctions.

- Lumbar Facet Joint -

A facet joint or zygapophyseal joint is a little articular joint that sits between 2 vertebrae and run through the entire spinal column. The orientation of the facet joints in the lumbar spine allows for flexion and extension to be the primary movements and with compressive load passing through the facet joints when in an extended position.

The facet joint is a synovial joint and can undergo change when loaded incorrectly. You can sprain a facet joint as you can your ankle joint and the facet joint can also display degenerative changes.

We often see those suffering with chronic low back pain avoiding flexion of the lumbar spine or really locking up the lower spine and hingeing at the hips to bend forwards. This places an increase load through the facet joints as well as a compressive load through the intervertebral discs and increasing the risk of irritation or development of osteoarthritis. As a result of this, our global musculature grips on to try provide a little extra support and stability, further reducing range of movement and increasing tension through the lower spine and hips.

Safely restoring lumbar flexion and extension movement will help significantly for those chronic stiff and achy low backs or repetitive facet sprains.

NB: when lifting with load/weight, there are different considerations in order to help protect the facet joints and intervertebral discs when going through these range of movements.
