Law of Electrolytes:

We are always confused about fluid electrolyte balance, which electrolyte when reduced and when increased, So sharing just a few simple ideas::

We know aldosterone/RAAS increase Na+, Water but lose K+

👉so, Those drugs which inhibit RAAS will act opposite to aldosterone(eg, ACEi, Spironolactone), also adrenocortical failure( eg, Addison's) But when adrenocortical activity increases, will work as aldosterone( Conns, Cushing)

👉Steroid has a negative balance in calcium, probably inhibit, Ca absorption from gut, so there is the same rule.

👉Diuretics, Frusemide and thiazide is best friend, cause, They show same effect in electrolyte balance, opposite to mannitol, which cause Osmotic diuresis, causing, Hypovolemic hypernatremia and hyperkalemia

👉Ca++, Mg++, Albumin, these three also the best friend, shows always unidirectional effect in a certain situations.

✍️Sympathetic opththalmitis:

এক চোখের Ciliary region এ injury হওয়ার অনেক দিন পর আরেক চোখ affected হবে।
70-80% within 3 months
90% within 1 year
62 year পরেও হওয়ার রেকর্ড আছে। S/S uveitis এর মতোই, তবে severe থাকবে।

Pathogenesis unknown, But may be an autoimmune hypersensitivity reaction to Ag in uveal tissue, uveal pigments or retinal antigen.

✍️✍️A fistula is an abnormal communication between two epithelial lined surfaces. This communication or tract may be lined by granulation tissue but may become epithelialized.

Although fistula is abnormal, but some fistula is not harmful, rather beneficial, Here are some examples of helpful fistula in picture

Fistula that are harmful---

* Congenital( Tracheo-oesophageal fistula, Branchial fistula, Congenital AV fistula, Umbilical fistula( Patent vitellointestinal duct)

* Acquired
👉Traumatic
💬Following surgery: Intestinal fistula(Biliary, pancreatic, Faecal)
💬Following instrumental delivery(VVF, RVF, Utereterovaginal fistula)
👉Inflammatory and infectious( Intestinal actinomycosis, TB, Fistula in ano)
👉Malignancy
💬Rectovesical fistula in ca re**um
💬Vesicouterine fistula in uterine cancer
