Looking for a Cardiac Nurse Practitioner to join our team in Bracebridge and Huntsville!
Reports to: Executive Director of Cottage Country Family Health Team and Director of Utilization & MAOHT Clinical Coordination at MAHC
Status: 3 days per week. 12-month contract with possibility of extension
Salary: $55.00 to $62.50 per hour, with vacation plus HOOPP
Hours of Work: Part Time
PATIENT CARE
✅Works within the legislated scope of practice of the Nurse Practitioner, determines the need for, orders and interprets reports of appropriate laboratory and diagnostic imaging.
✅Collaborates with hospital internists and local primary care providers to enable comprehensive care and referral for congestive heart failure patients within Muskoka and Area.
✅Conducts patient assessments and develops care plans for all referred CHF patients triage as appropriate to other care settings.
✅Provides direct comprehensive care to patients with specialized and complex needs.
✅Ongoing management of HF patients (NYHA Class II and III)
✅Consultation and referral as appropriate to the RVH consultant and HF Clinic (Node)
✅Performs the responsibilities of the position consistent with the operational policies of the CCFHT.
