NEW PROGRAM LAUNCH

We’re excited to share that we’re launching a brand new 9-week In Person series called Living Well with the Perth Family Health Team!

Starting Tuesday, April 14 and running until June 9, this series will feature a variety of PFHT healthcare providers and specialists, with a different presenter each week.

Each session will focus on a new topic and provide practical, evidence-based tips and information to help support your overall health and well-being.

Think of it as a chance to learn something new, ask questions, and pick up helpful strategies you can use in everyday life — all from members of your PFHT care team.

Spring Tulip Sale for Parkinson’s Awareness!!

April is Parkinson's Awareness Month and the tulip is the international symbol of Hope for Parkinson's Disease. We'll be out Saturday March 28th spreading hope at several locations in Perth, Smiths Falls, Carleton Place, Merrickville and Westport with beautiful potted tulips for sale. They will be ready to take home and brighten your home and brighten the lives of those affected by Parkinson's. $12 each pot, 10 am until sold out. We will be able to accept tap for donations as well. In Smiths Falls, (YIG 25 Ferrara Drive) Perth (YIG 80 Dufferin Street), Carleton Place (FreshCo 110 Lansdowne Avenue), Merrickville (Food Market 205 St. Lawrence Street) and Westport (Kudrinko's 22 Main Street). Dr. Van Noppen was recently recognized through our Gifts of Gratitude program by a very thankful patient.

What an Incredible Senior Social Connection Week! 💛

A big thank-you to everyone who joined us for The Great Indoor Backyard Party! We played games, enjoyed some delicious food, shared plenty of laughs, and even welcomed a few new faces — which made it all the more special.
