I’ve been seeing a pattern in my clients leading up to hip surgery:

reduced range of motion in both flexion and hip rotation. Here’s why:⁠ ⁠

🔎 Hip degeneration (often due to arthritis) makes the joint stiff and limits mobility, especially in deep squats and rotational movements. ⁠ ⁠

💥 Muscle imbalances and pain avoidance also play a role. Tightness and compensations make movement even more restricted. ⁠ ⁠

🧠 If you’re struggling with this, working on mobility and strength in a safe range can make a big difference and potential mitigate the need for surgery!⁠ ⁠

Shoulder impingement is a common condition characterized by pain and inflammation in the shoulder joint. It occurs when the rotator cuff tendons and the subacromial bursa become compressed or pinched between the acromion (a bony projection of the shoulder blade) and the humeral head (the upper arm bone). This compression can lead to irritation, inflammation, and pain, especially with overhead activities or movements that involve raising the arm.

📝 Common Assessments for Shoulder Impingement:

✅ Painful Arc Test:
The painful arc test is a simple assessment used to determine shoulder impingement. To perform this test, the patient is asked to actively raise their arm from their side to overhead in a smooth motion. The examiner observes for any pain or discomfort experienced by the patient during this movement. Pain typically occurs between 60 to 120 degrees of shoulder abduction and may indicate impingement of the rotator cuff tendons between the acromion and humeral head.

✅ Neer Test:
The Neer test is another assessment used to determine shoulder impingement. In this test, the examiner stabilizes the scapula with one hand while passively internally rotates the patient’s arm and flexes the shoulder with the other hand. This maneuver brings the greater tuberosity of the humerus into contact with the undersurface of the acromion, compressing the rotator cuff tendons and subacromial bursa. The test is considered positive if the patient experiences pain or discomfort in the shoulder during the movement.

✅ Hawkins-Kennedy Test:
The Hawkins-Kennedy test is a provocative maneuver used to assess for impingement of the supraspinatus tendon. To perform this test, the examiner flexes the patient’s shoulder to 90 degrees and then forcibly internally rotates the arm. This motion brings the greater tuberosity of the humerus into contact with the coracoacromial ligament and acromion, potentially impinging the supraspinatus tendon. A positive test is indicated by pain or discomfort felt by the patient during internal rotation of the shoulder.
