The objective this study was to propose a sequential MRI SIJ followed by MRI spine only for those patients who do not have inflammatory lesions on MRI SIJ, to optimise MRI use and save resources.

The findings of this retrospective study show that MRI of the sacroiliac joints alone was diagnostic in 82% of patients, with only 18% requiring additional spinal MRI. A sequential approach could have saved £11,118 and 34 hours of scanner time in our cohort.

Inflammatory bowel disease may predict the need for spinal MRI, while other SpA features did not.

This study analysed information from IMAS - the International Map of Axial Spondyloarthritis. In a nutshell, the global delay to diagnosis is 7.4 years with large regional differences. What causes a longer delay to diagnosis? Younger age at symptom onset, female gender, diagnosis made by a rheumatologist, greater number of HCPs seen before diagnosis and history of uveitis were the parameters associated with a longer diagnostic delay in axSpA patients.

In this global sample of axSpA patients the mean diagnostic delay was 7.4 years, and showed significant differences across regions.
