Recent IPAC lapses reported across medical facilities highlight serious risks to patient safety, including inadequate instrument reprocessing, failure to properly clean, disinfect, and sterilize reusable medical devices, and the use of unsterile needles with multidose vials. These gaps can lead to cross-contamination and potential exposure to blood-borne viruses.

Following Manufacturer’s Instructions for Use, proper disassembly of instruments, use of autoclaves, adherence to Medical Device Reprocessing CSA Z314 standard and PIDAC Best Practices, proper use of multidose vials, safe preparation and use of syringes, and having documented IPAC policies and procedures are just some of the essential IPAC practices required to reduce infection risks.
