Stochastic vs. Deterministic Effects — What Radiation Really Does!
Understanding radiation effects isn’t just physics…
It’s the foundation of radiation safety in Nuclear Medicine.

✅ Stochastic Effects
“Chance-based effects — no threshold.”
Even small doses can cause them, but probability increases with dose.

• Caused by random DNA mutations
• Severity does NOT depend on dose
• Only the probability increases
• Examples: Cancer induction, genetic mutations

In Nuclear Medicine:
These effects guide our principle of ALARA — keeping dose “As Low As Reasonably Achievable”.

✅ Deterministic Effects
“Threshold effects — dose must cross a limit.”
Below the threshold → No effect
Above the threshold → Effect appears and severity increases.

• Caused by cell death in large numbers
• Severity increases with dose
• Examples: Skin erythema, cataracts, sterility, ARS (Acute Radiation Syndrome)

In Nuclear Medicine:
Deterministic effects are rare due to low doses — but critical in therapy doses, spills, and contamination events.

Ischemia vs. Infarct — The Difference Your Scan Can See!

Ischemia = “Struggling Heart Muscle”
Blood flow is reduced but not completely blocked.
The tissue is still alive, still salvageable — but signalling distress.

Infarct = “Dead Heart Muscle”
Blood flow has been completely cut off for too long.
Muscle becomes non-viable and cannot recover.



How Nuclear Medicine Differentiates Them

Ischemia = Reversible Defect
• Normal uptake at rest
• Reduced uptake during stress
→ Indicates treatable disease (angioplasty/stent may help).

Infarct = Fixed Defect
• No uptake in both rest & stress
→ Indicates permanent damage (scar).

Why This Matters Clinically

• Helps cardiologists decide who needs a stent
• Predicts viability and potential recovery
• Prevents unnecessary invasive procedures
• Guides management in chronic coronary artery disease

Running for the Heart — The Science of Physical Stress Testing

When we make patients walk or run on a treadmill during a cardiac scan, we’re not just testing stamina — we’re testing the heart’s true performance under pressure.

Physical Stress Test helps reveal hidden ischemia — the areas where blood flow fails to meet demand when the heart is working hard.

How it works:
•The patient exercises on a treadmill (or cycle) while ECG, blood pressure, and symptoms are monitored.
•Once the target heart rate is achieved, a radiotracer (like ⁹⁹ᵐTc-sestamibi) is injected.
•Myocardial perfusion imaging (MPI) then captures how well different regions of the heart are being perfused.

What it tells us:
•Normal perfusion = healthy coronary arteries
•Reduced perfusion = possible blockage

Sometimes, the heart’s silence during rest speaks only when it’s under stress.
