🔹Neural mobilization is a movement-based therapy, applied manually or via exercise. The nerve is mobilized relative to adjacent structures, with the aim of reducing symptoms through mechanisms that may be mechanical, and improved load tolerance, or neurophysiologic.

🔹The femoral nerve is located in the pelvis and goes down the front of the leg. It helps the muscles move the hip and straighten the leg. It provides feeling (sensation) to the front of the thigh and part of the lower leg.

🔹Graded exposure stretching, whether performed independently or by the manual therapist appears to be helpful in relieving femoral nerve compression due to lumbar spine pathology (L2-L4) or musculotendinous entrapment.

One thing that really bugs me is that simple answers to complex problems are often presented that have NO real basis in truth

The diagram in the picture is from a recent back pain paper "Can Biomechanics Research Lead to More Effective Treatment of Low Back Pain?” Cholewicki 2019 JOSPT & is "A metamodel illustrating factors (colored circles) contributing to low back pain, disability, quality of life, and other outcomes”. I like the diagram but don’t agree with all in the paper.

This highlights nicely the complexity of the problem of low back pain. So when you read about that one stretch or muscle that needs to be activated or some other simplistic singular answer. Please have a chuckle with me

🔹We usually think of pain as being simple, for example, you sprain your ankle and now you have pain. It makes sense to assume if you hurt or damage your body, then you will have pain.

🔹But what about those times you have pain without a cause? People often say they have pain, but can't recall a specific injury. Pain is not determined by a specific injury that happens to a tissue. How you experience pain and to what level is an uconscious interpretation by the brain.

🔹The biopsychosocial approach describes pain and disability as a multidimensional, dynamic interaction among physiological, psychological, and social factors that reciprocally influence one another, resulting in chronic and complex pain syndromes.

You might see it referred to as spinal traction.
