Key Responsibilities

1. Conduct psychosocial assessment and case studies of patients and their families.

2. Provide counseling, emotional support, and crisis intervention to patients in need.

3. Facilitate discharge planning and coordinate referrals to community resources and support services.

4. Process claims for social assistance programs such as PhilHealth, PCSO, DSWD, LGU aid, and MAIFIP for indigent patients. Or, link patients to appropriate agencies and programs for financial, medical, and social support.

5. Advocate for the protection of patients’ rights and ensure confidentiality of information.

6. Maintain accurate and confidential social service records, case reports, and required documentation.

7. Collaborate with doctors, nurses, and other health professionals for holistic patient care.

8. Implement and support hospital social service programs in line with DOH and DSWD guidelines.

9. Other Duties
Perform other related duties as may be assigned by the supervisor or management, consistent with the organization’s goals and objectives.

Qualifications

• Education:
o Bachelor’s degree in Business Administration, Office Administration, Public Administration, Pscyhology/Hr Management, Accounting, or any related field.

• Experience:
o At least 1–3 years of relevant work experience in administration, HR, or finance, experience in a hospital, clinic, or healthcare setting is an advantage but not required
o Experience in handling personnel records, payroll, permits, and in managing/accounting funds

• Knowledge, Skills, and Abilities:
o Proficiency in MS Office applications (Word, Excel, PowerPoint) and familiarity with biometric attendance systems.
o Strong organizational, documentation, and record-keeping skills.
o Ability to manage revolving funds, liquidations, and other financial transactions.
o Skilled in internal and external communication, including documentation of meetings and official correspondence.
o Ability to coordinate with training institutions, government offices, and external stakeholders.

Key Responsibilities

1.Clinical Care and Patient Management
Perform nursing assessments, monitor vital signs, and record patient conditions accurately.
Administer medications, treatments, and IV therapy per physician’s orders.
Assist in medical procedures and coordinate with attending physicians.
Ensure proper documentation of care in patient charts and electronic records.
Monitor patient progress and promptly report any significant changes to the physician.

2.Emergency and Infirmary Response
▪︎Respond to emergencies and perform basic life support (BLS) and first aid as needed.
▪︎Assist in minor surgical procedures and provide wound care.
▪︎Prepare and assist in the transfer of patients to higher-level health facilities when necessary.
Implement infection prevention and control measures.
▪︎Ensure proper use of PPE and maintenance of a clean and safe environment.

3. Patient Education and Advocacy
▪︎ Educate patients and families on health maintenance, disease prevention, and treatment adherence.
▪︎ Promote maternal and child health, nutrition, hygiene, and vaccination programs.

4. Coordination and Reporting
▪︎Coordinate with midwives, community health workers, and other health personnel for integrated patient care.
▪︎Maintain accurate inventory of supplies, request needed items, and avoid wastage.
▪︎Submit nursing reports and participate in case conferences and staff meetings.

5. Other Duties

- Regular patrols of the premises, both internal and external.
