حالة جديدة من عيادة الامراض الصدرية:
مريض ٧٤ سنة مدخن شره ، بدأ يشتكى من ألم في جنبه الشمال من البطن فعمل سونار على البطن وطلع عنده ورم في الطحال.
ماعملش فحوصات بعدها عشان ظروفه المادية.
بعد ب ٥ شهور رجع وهو بيعاني من نهجان وكحة وألم شديد فى الصدر من الناحية الشمال.
الاشعة المقطعية على الصدر وضحت ورم فى منتصف الصدر وورم و كسر فى الضلع الخامس الأيسر بالرغم ان مكنش عنده اى تاريخ مرضى من اصابة او كدمات فى الصدر او الضلوع.
حالة المريض لم تسمح بإجراء منظار شعبى .
فقررنا ناخد عينة من الورم اللى موجود فى الضلع المكسور.
أخدنا عينة باستخدام سونار الصدر تحت بنج موضعي → والنتيجة سرطان بالرئة منتشر بالضلوع.
✨ الرسالة:
• مش أي كسر في الضلوع يبقى خبطه أو حادثة ، أوقات يكون الكسر نتيجة لانتشار ورم بالعظام .
• ممكن يكون انتشار الاورم بالبطن او العظام هو العلامة الاولى اللى تخلى المريض يشتكى .
• استخدام سونار الصدر فى حالات كتير بيساعدنا فى التشخيص بأمان بدون الحاجة لتخدير او منظار على الرئة
A 74-year-old COPD male patient started complaining of left upper quadrant abdominal dull aching pain 5 months ago. Ultrasound abdomen showed a splenic focal lesion. Outside our hospital, he underwent a splenic biopsy❗️which unfortunately was complicated by laceration → urgent splenectomy. Pathology: malignancy, likely metastatic.
Unfortunately, due to financial constraints, he did not pursue further investigations
Four months later, he presented to our outpatient clinic with progressive dyspnea, cough and stabbing chest pain on the left side.
CT chest revealed a mediastinal mass and a pathological rib fracture.
The patient was too hypoxic for bronchoscopy.
So, we went for a safer option: ultrasound-guided tru-cut biopsy from the rib lesion under local anesthesia.
🔬 Pathology confirmed: adenocarcinoma, metastatic.
💡 Take-home message:
• Splenic focal lesions can be the first clue of an underlying systemic malignancy.
• Pathological rib fractures = always think of metastasis.
• When the patient is too fragile for invasive procedures, image-guided biopsy can be a life-saving diagnostic tool.
Medicine is full of surprises — sometimes the rib tells the story the spleen began
