This patient presented with the classic warning sign: sharp pain upon cold application that subsided immediately upon removal of the stimulus. (Diagnosis: classic reversible pulpitis).

The Culprit: A large, defective composite that clearly gave up the fight a long time ago. (Note to self: I was lazy and skipped the initial photo. Oops! 🤦‍♂️)

The Battle: Rubber dam application was a fight today—one of those cases where the tooth just doesn't want to cooperate. But thanks to our hero, the B4 CLAMP, we secured perfect isolation and won the war! 😉

The Protocol:
Used Caries Detecting Dye by Ultradent (Seek) to meticulously trace the lesion and establish a clean Peripheral Seal Zone. We went deep, but kept that margin clean to avoid an unwelcome exposure.

Visit 1: Deep Margin Management. The Biobase was established using a bond and flowable composite layer, and Deep Margin Elevation was completed on the buccal wall. Patient dismissed to let the bond decouple (stress relaxation) and—let's be honest—for my back pain to go away! 😅

Visit 2: Rubber dam back on (much easier this time!), and the final direct composite restoration was placed.

That's how you manage a deep case with precise staging and keep your sense of humor. 😇
