Recessions in the 5th sextant can be particularly challenging to manage. Due to each patient’s unique anatomical conditions, achieving complete root coverage is not always possible. In this case, I opted for a tunnel technique to obtain the best possible esthetic and biological outcome.

Challenging lower incisors root coverage case successfully treated!

This patient presented with severe gingival recessions in the lower incisors, combined with a thin periodontal biotype and significant soft tissue loss — a situation that made the case particularly complex.

To address this, I used a modified tunneling technique combined with the “zipper” approach, allowing for precise tissue handling and tension-free coronal advancement. The result: excellent root coverage, improved tissue thickness, and a very satisfied patient.

Mastering soft tissue management is key in periodontal plastic surgery — and it’s always rewarding to see these transformations come to life.

On March 28th and 29th, we had the pleasure of hosting this international course at Odos Dental, with the participation of doctors who traveled to Madrid to enjoy two intensive days of learning and hands-on experience. The program included lectures and clinical updates on immediate implants, bone regeneration, and soft tissue management, as well as a practical workshop to apply these techniques and a live surgery. Two days full of education, clinical excellence, and great moments in Madrid.

The subepithelial connective tissue pedicled split palatal flap for soft tissue coverage on extraction sites was first published by Wang in 1993 and then by Nemcovsky and Artzi in 1999.
This flap consists of a strip of connective tissue that is separated from the palatal vault and then rotated over the defect.
The donor site remains primarily covered thanks to the remaining epithelial portion of the palate flap.
This flap increases both the height and width of the soft tissue at the recipient site.
This approach is advantageous for various reasons.
First, there is a direct blood supply from the base of the pedicle, which reduces the risk of necrosis.
Second, the esthetics achieved with this flap is good.
Third, the procedure is relatively simple, requiring only an accurate fixation on the buccal part of the crest.
Fourth, there is little morbidity.
Finally, the flap increases the soft tissue at the recipient site both in horizontal and vertical dimensions.
Success with the palatal flap depends on the thickness of the palatal mucosa, which is greatest between the canines and molars.
The flap can be done from premolars rotating to anterior zone or from canine rotating to premolar or molar zone, only changes the blood supply that comes from nasopalatine artery first or greater palatine artery in the second variant of the technique.
Another important factor is the distance from the gingival margin to the palatine neurovascular bundle which varies from 7 to 17 mm at second premolar.

The resorptive changes of alveolar bone are an inevitable process following tooth loss.

Guided Bone Regeneration (GBR) is a successful modality for bone augmentation with a wide range of indications and helps restore the alveolar ridge dimensions.

It utilizes the principle of Guided Tissue Regeneration (GTR) for space maintenance within a bony defect.

Different types of barrier membranes are being utilized along with various bone grafts in GBR.

Thorough knowledge regarding the biology of bone is required before the initiation of any bone augmentation procedure.

In this case, bone regeneration was performed with a mixture of autologous bone and allograft and a PTFE membrane. and here we have the result after 6 months of healing at the time of placing the implants
