Tapeworm infection cause, treatment and prevention

Tapeworm infection cause, treatment and prevention Adult tapeworms, which live in people's intestine, usually cause no symptoms, but they may cause abdominal discomfort, diarrhea, and weight loss.

The pork tapeworm can also form cysts in the brain and elsewhere in the body (called cysticercosis). Adult tapeworms of other species—Echinococcus granulosus and Echinococcus multilocularis (dog tapeworms)—live in the intestine of dogs or other canines. These tapeworms sometimes infect people, causing cysts in the liver or other organs. Tapeworms have three main parts:

The head, which is the part that attaches to intestine
The neck, which can regenerate
The rest of the worm, which consists of many segments (called proglottids) and which may contain eggs
If treatment does not eliminate the head and neck, the entire worm may re-form. Fish Tapeworm
Fish Tapeworm
(FROM GC COOK, MANSON'S TROPICAL DISEASES, 20 ED, 1996, WB SAUNDERS, FIG. 74.11; WITH PERMISSION.) Pork, beef, and fish tapeworm life cycle
The pork, beef, and fish tapeworms are large, flat, ribbon-like worms that live in the intestine of people and can grow 15 to 30 feet (4.5 to 9 meters) in length. (People are considered the definitive host because adult tapeworms live in their intestine.) Egg-bearing sections of the worm (proglottids) are passed in the stool (feces). If untreated human waste is released into the environment, the eggs may be ingested by intermediate hosts, such as pigs, cattle, or, in the case of fish tapeworms, small freshwater crustaceans, which are in turn ingested by fish. The eggs hatch into larvae in the intermediate host. The larvae invade the intestinal wall and are carried through the bloodstream to skeletal muscle and other tissues, where they form cysts. People acquire the parasite by eating the cysts in raw or undercooked meat or certain types of freshwater fish. The cysts hatch and develop into adult worms, which latch onto the wall of the intestine. The worms then grow in length and begin producing eggs. Life Cycle of the Pork Tapeworm
1. People may become infected when they eat raw or undercooked pork containing cysts of tapeworm larvae (called cysticerci).
2. In the intestine, the cysticerci mature into adult tapeworms and attach themselves to the wall of the intestine.
3. Adult tapeworms produce segments (called proglottids) that bear eggs. The proglottids may release the eggs or detach from the rest of the tapeworm and travel to the anus.
4. The eggs, proglottids, or both are passed in stool.
5. Pigs or, less often, people become infected by consuming the eggs or proglottids (for example, in food contaminated with human stool).
6. After the eggs are consumed, they hatch in the intestine and release spheres (called oncospheres) that penetrate the wall of the intestine.
7. The oncospheres then travel through the bloodstream to muscles and to the brain, liver, and other organs, where they develop into cysts.

Cattle are intermediate hosts for T. saginata. Humans are infected by

Eating cysticerci (larval form) in raw or undercooked beef
The larvae mature in about 2 months to adult worms that can live for several years; usually, only 1 or 2 adult worms are present. Adult T. saginata tapeworms are usually 4 to 12 meters in length, but can be as long as 25 meters.

T. saginata infection occurs worldwide but especially in cattle-raising regions of the tropics and subtropics in Africa, the Middle East, Eastern Europe, Mexico, and South America. Infection is uncommon in US cattle and is monitored by federal inspection.

Symptoms and Signs of Taenia saginata Infection
Patients may be asymptomatic or have mild digestive symptoms including epigastric discomfort, nausea, flatulence, diarrhea, or hunger pains. Passage of a motile segment (proglottid) often brings an otherwise asymptomatic patient to medical attention.

Diagnosis of Taenia saginata Infection
Microscopic examination of stool for ova and proglottids
The stool should be examined for proglottids and ova; ova may also be present on a**l swabs. The ova of T. saginata are indistinguishable from those of T. solium (pork tapeworm) and T. asiatica, as are the clinical features and management of intestinal infections due to these 3 tapeworms.

Treatment of Taenia saginata Infection
Praziquantel
Alternatively, niclosamide (outside the US)
Treatment of T. saginata infection is with a single oral dose of praziquantel 5 or 10 mg/kg.

Alternatively, a single 2-g dose of niclosamide (not available in the US) is given as 4 tablets (500 mg each) that are chewed one at a time and swallowed with a small amount of water. For children, the dose of niclosamide is 50 mg/kg (maximum dose 2 g) once.

Treatment can be considered successful when no Taenia ova are identified in stool 1 and 3 months after treatment.

T. saginata infection can be prevented by cooking whole cuts of beef to ≥ 63° C (≥ 145° F) as measured with a food thermometer placed in the thickest part of the meat, then allowing the meat to rest for 3 minutes before carving or consuming. Ground beef should be cooked to ≥ 71° C (≥ 160° F). Ground beef does not require a rest period.

Treatment of Taeniasis
Two drugs are effective for the treatment of human taeniasis, niclosamide and praziquantel (PZQ). Niclosamide (2 g orally in a single dose for adults) is safe and well tolerated, with only mild and transient side effects. Its efficacy in treatment series has been reported to be above 80% (70). In a large-scale community-based deworming program, however, its efficacy was only 67% (71). Praziquantel seems to be effective at either 5 or 10 mg/kg (also a single oral dose) (72). If a person has taeniasis and simultaneously has latent, asymptomatic neurocysticercosis, praziquantel treatment for taeniasis may affect previously silent brain cysts and trigger seizures or other neurological symptoms (73–75). Judging by the numbers of doses of praziquantel used for schistosomiasis in regions of cysticercosis endemicity in Africa, this event seems to be extremely uncommon, but the impact of newly developed seizures or neurological symptoms in one or a few individuals could still be devastating for a community-based control program. On the other hand, only a few studies with mass chemotherapy for schistosomiasis were designed with sufficient follow-up to detect adverse central nervous system (CNS) events developing days after treatment. Niclosamide is not absorbed from the intestinal tract and thus it does not affect brain cysts. The reported efficacy of benzimidazoles such as mebendazole and albendazole (ABZ) to treat taeniasis in single-dose regimes is low (76, 77), requiring multiple doses and several days of treatment (78–81).

Taenia solium is another parasite that causes health problems. It enters the body when we eat undercooked pork that contains tapeworm larvae. A larva attaches itself to the wall of the small intestine and grows into an adult. The tapeworm survives by absorbing food that we've digested.

If we ingest tapeworm eggs, cysts containing larvae may appear in our brain as well as in other parts of our body. A larval tapeworm is known as a cysticercus. The presence of larval cysts in the brain is known as neurocysticercosis. The cysts may cause seizures, epilepsy, and other serious problems.

A widely used drug against tapeworm infections is found to inhibit colon cancer metastasis.
Colon cancer is one of the most common tumor diseases in Western countries. In Germany alone, there are approximated 73 000 new cases of the disease every year. Despite surgery, chemotherapy and radiation therapy, only about half of the affected patients are cured.

Treatment
Some people with tapeworm infections never need treatment, for the tapeworm exits the body on its own. Others don't realize they have it because they have no symptoms. However, if you're diagnosed with intestinal tapeworm infection, medication will likely be prescribed to get rid of it.
Treatments for intestinal infections
The most common treatment for tapeworm infection involves oral medications that are toxic to the adult tapeworm, including:
Praziquantel (Biltricide)
Albendazole (Albenza)
Nitazoxanide (Alinia)

It’s not uncommon these days to find schools that have declared they are “nut-free” That means the onetime staple of kids’ lunchboxes — a peanut butter and jelly sandwich — is nowhere to be found on school grounds. That’s because peanuts can cause a life-threatening reaction in some people. Peanuts are one of the food allergens most commonly associated with anaphylaxis, a sudden and potentially deadly condition that requires immediate attention and treatment.
In recent years, awareness about peanut allergy in children has risen, as has the number of peanut allergy cases reported. A 2017 study reported that peanut allergy in children had increased 21 percent since 2010, and that nearly 2.5 percent of U.S. children may have an allergy to peanuts. There are several misconceptions about peanut allergies. For example, a peanut is a legume (belonging to the same family as soybeans, peas and lentils), not a tree nut. And while it was previously believed that an allergy to peanuts was lifelong, research has shown up to 20 percent of individuals with a peanut allergy eventually outgrow

Sometimes you don’t need to do anything about a tapeworm. It could leave your body on its own. But if your doctor finds it, they can prescribe a medicine like praziquantel or nitazoxanide. These will either kill the adult worms or cause you to p**p them out. But they won’t kill the eggs, which can still cause infections. You’ll likely need to give your doctor a stool sample for a few months to make sure all the worms are gone.
It’s harder to treat an infection caused by tapeworm cysts. In addition to the medicine that kills the tapeworm, you may need medicine to reduce inflammation or other symptoms, like seizures, that you’re having. Depending on where your cysts are and how many you have, you may need surgery to remove them.
How Do You Prevent Worms?
To avoid being infected with either pinworms or tapeworms, it’s crucial that you wash your hands with soap and water before eating or handling food and after going to the bathroom or changing diapers.
Here are some other ways to prevent them:
Keep your fingernails clipped short.
Take a shower or bath every day (in the morning is best).
Try not to scratch around your a**s or between your legs.
Wash your clothes and bed linens often.
To help prevent tapeworms, try these things:
Don’t eat raw or undercooked beef, pork, or fish.
If you have a dog with tapeworm, treat it right away.

Tapeworms are a type of parasite that live in the intestines of animals. They have a head, neck, and many body segments called proglottids. They lay eggs, which then hatch into larvae before maturing into an adult tapeworm.
Humans can become infected by eating eggs or larvae in contaminated water or meat. The infection in humans is usually easily treated but sometimes it can cause serious problems.
If a human or an animal consumes tapeworm eggs, the eggs hatch into larvae, which break through the wall of the intestines and grow inside cysts in tissues or organs. These can cause life-threatening damage.
If you consume tapeworm larvae, they can develop into adult tapeworms in the intestines. They attach to the wall of the intestine with suckers or hooks, and remain there to feed and grow. An adult tapeworm can be very long and live in the intestine for many years.
The most common tapeworm infection in Australia is caused by dwarf tapeworm. There is also a risk in Australia of contracting a serious condition called hydatid disease from the eggs of a type of tapeworm called Echinococcus granulosus found in dog faeces (poo). You should see your doctor if you have any signs of tapeworm infection.
It is also possible to be infected by other types of tapeworm overseas.
What are the symptoms of tapeworm infection?
People with tapeworm infection often have no symptoms. If you have symptoms, they will depend on the type of tapeworm and where it is in your body.
Intestinal tapeworm infection can cause:
pain in the abdomen (tummy)
nausea
weakness or tiredness
diarrhoea
loss of appetite
weight loss
vitamin and mineral deficiencies
pieces of worm in the faeces
If you develop a complication you may have:
fever
allergic symptoms
headaches, dizziness or seizures
If you have hydatid disease, you may have fluid-filled cysts in the body. These most commonly grow in the liver or lungs, but can grow in any organ including the brain, heart or bones. The cysts normally do not cause problems unless they grow very large or break.
What causes tapeworm infection?
You can be infected with tapeworm larvae by eating raw or undercooked meat, such as beef, pork, or fish.
You can consume tapeworm eggs in infected water or by accidentally coming into contact with dog faeces, for example by handling an object, food or water that has been contaminated.
How is tapeworm infection diagnosed?
Your doctor will ask for a stool sample and perhaps a blood test to confirm that you have tapeworms and what kind of tapeworm you have.
If you have hydatid disease, you may need an x-ray, ultrasound, or MRI scans to diagnose the cysts.
How is tapeworm infection treated?
You will probably be prescribed medicine to kill the tapeworms.
If you have hydatid disease, you may need other medication and perhaps surgery to remove the cysts.
Can tapeworm infection be prevented?
You can reduce your risk of tapeworm infection by:
washing your hands after using the toilet and before handling food
following food handling guidelines
not eating undercooked pork, beef and fish
controlling tapeworm infection in your dog, with preventive deworming and quickly treating any dog infected with tapeworm
not letting a dog lick your face
not feeding dogs raw offal
Tapeworms are flat, segmented worms that live in the intestines of some animals. Animals can become infected with these parasites when grazing in pastures or drinking contaminated water.
Eating undercooked meat from infected animals is the main cause of tapeworm infection in people. Although tapeworms in humans usually cause few symptoms and are easily treated, they can sometimes cause serious, life-threatening problems. That's why it's important to recognize the symptoms and know how to protect yourself and your family.
Tapeworm Causes
Six types of tapeworms are known to infect people. They are usually identified by the animals they come from -- for example, Taenia saginata from beef, Taenia solium from pork, and Diphyllobothrium latum from fish.
Tapeworms have a three-stage lifecycle: egg, an immature stage called a larva, and an adult stage at which the worm can produce more eggs. Because larvae can get into the muscles of their hosts, infection can occur when you eat raw or undercooked meat from an infected animal.
It is also possible to contract pork tapeworms from foods prepared by an infected person. Because tapeworm eggs are passed with bowel movements, a person who doesn't wash hands well after wiping and then prepares food can contaminate the food.
Tapeworm Symptoms
Sometimes tapeworms cause symptoms such as
Nausea
Weakness
Diarrhea
Abdominal pain
Hunger or loss of appetite
Fatigue
Weight loss
Vitamin and mineral deficiencies
However, often tapeworms don't cause symptoms. The only sign of tapeworm infection may be segments of the worms, possibly moving, in a bowel movement.
In rare cases, tapeworms can lead to serious complications, including blocking the intestine, or smaller ducts in the intestine (like the bile duct or pancreatic duct).
If pork tapeworm larvae move out of the intestine, they can migrate to other parts of the body and cause damage to the liver, eyes, heart, and brain. These infections can be life-threatening. Seizures are the most common symptom of cysticercosis, the infection caused by the larvae of the pork tapeworm.

If worms are not detected in the stool, your doctor may order a blood test to check for antibodies produced to fight tapeworm infection. For serious cases, your doctor may use imaging tests such as computed tomography (CT) or magnetic resonance imaging (MRI) to check for tapeworm lesions or secondary swelling outside of the digestive tract.
The type and length of treatment may depend on the type of tapeworm you have. Tapeworms are usually treated with a medicine taken by mouth. The most commonly used medicine for tapeworms is praziquantel (Biltricide).
These medications paralyze the tapeworms, which let go of the intestine, dissolve, and pass from your body with bowel movements. If worms are large, you may have cramping when they pass. Your doctor will recheck stool samples at one and three months after you finish treatment. When tapeworms are confined to the intestines, appropriate treatment gets rid of them in more than 95% of people.
More serious complications of tapeworm infection are also treated with medications.
