❇️ Common Types of Abdominal Wall Hernias:

▶️ Epigastric Hernia

* Protrusion of preperitoneal fat or abdominal contents through a defect in the linea alba between the xiphoid process and umbilicus.

* Often small, may be painful, and usually contains fat rather than bowel.

▶️ Umbilical Hernia

* Herniation through a defect in the umbilical ring.

* Common in infants, pregnancy, obesity, and ascites.

* In adults, it has a higher risk of strangulation than in children.

▶️ Incisional Hernia

* Occurs at the site of a previous surgical scar due to weakness of the abdominal wall.

* Risk factors include wound infection, obesity, poor nutrition, and increased intra-abdominal pressure.

▶️ Spigelian Hernia

* Herniation through the Spigelian fascia (along the semilunar line), typically lateral to the re**us muscle.

* Often interparietal and may be difficult to diagnose clinically.

* Has a high risk of strangulation.

▶️ Inguinal Hernia

* Protrusion through the inguinal canal, above and medial to the p***c tubercle.

* Most common type of hernia, especially in males.

* Can be direct or indirect.

▶️ Femoral Hernia

* Herniation through the femoral canal, below and lateral to the p***c tubercle.

* More common in females.

* Has a high risk of incarceration and strangulation.

🗣️Slenczka, who was involved in the first isolation of Marburg virus in 1967, describes the desperate hunt of the causative agent of this first filovirus disease outbreak in the center of Europe, its successful isolation, the likely route of transmission from a monkey trading station to vaccine production facilities.
Marburg virus disease (MVD) starts with sudden, severe symptoms like high fever, chills, and headache. It progresses to include nausea, vomiting, diarrhea, rash, abdominal and chest pain, and jaundice. In severe cases, it can lead to bleeding, delirium, liver failure, and shock.
The incubation period is 2 to 21 days.
Early symptoms (2–5 days after onset)
Sudden high fever
Severe headache
Chills
Muscle and joint pain
Malaise
Later symptoms (3–5 days after onset)
Nausea, vomiting, and diarrhea
Abdominal pain and cramping
Chest pain
Sore throat
Rash (flat and raised, often on the trunk)
Eye irritation and redness
Hiccups
Severe symptoms (can develop later)
Jaundice (yellowing of the skin and eyes)
Severe weight loss
Swelling and pain in the stomach area

Treatment:- There is no approved treatment for Marburg virus disease, so care is primarily supportive, focusing on rehydration, oxygen support, and managing symptoms like fever and pain. Early, intensive supportive care, including **intravenous fluids and electrolyte replacement, can significantly improve survival rates. Several experimental treatments, such as **candidate monoclonal antibodies and antiviral drugs, are under development but are not yet available.

Supportive care
Fluid and electrolyte management: This is crucial to combat dehydration caused by the illness, often administered through intravenous fluids.
Symptom control: This includes treating fever with acetaminophen and managing nausea and vomiting with drugs like **dimenhydrinate or ondansetron.
Oxygen therapy: Providing supplemental oxygen is important to maintain oxygen levels.
Blood and blood product transfusions: These may be necessary to manage blood loss and support the patient.
Experimental treatments under development
Monoclonal antibodies: Studies have shown promise in animal models, and they are being considered for human use.
Antivirals: Several antiviral drugs are in development, including galidesivir. Other antivirals like favipiravir and remdesivir have shown potential in non-human primates.
Important considerations for prevention and isolation
Early isolation: Patients suspected or confirmed to have MVD should be isolated to prevent further spread.
Strict precautions: Healthcare workers use strict precautions, including personal protective equipment (PPE), when caring for patients to avoid infection.
Note: Symptoms are similar to other severe diseases like malaria and typhoid, so a proper diagnosis is crucial.

Memoriam: Professor Amezene Tadesse Robelie (MD, MHPED, FCS ECSA, FACS)
College of Surgeons of East, Central and Southern Africa (COSECSA)
It is with deep sorrow and profound sadness that we announce the passing of Professor Amezene Tadesse Robelie, a respected African surgeon, educator, researcher and long-serving Fellow of the College of Surgeons of East, Central and Southern Africa (COSECSA).
Professor Amezene was a distinguished pediatric surgeon, a dedicated trainer and examiner for COSECSA for more than fifteen years, and an exemplary leader who served as Panel Head of the Pediatric Surgery Program for more than 6 years. He also served the college as a certified Basic Sciences Master trainer, a member of the Editorial Board of the East and Central African Journal of Surgery and member of the court of examiners of the college.
Professor Amezene was widely admired and respected for his integrity, humility, and professionalism; he was an always dependable Fellow, a trusted colleague, and a true asset to COSECSA and to the surgical fraternity across Africa.
On behalf of the President of the College, the Council, and the entire COSECSA Fellowship, I extend our heartfelt condolences to our fellow Ethiopian colleagues, to his family, friends, and all who had the privilege of working alongside him.
Professor Amezene’s legacy as a teacher, mentor, and compassionate surgeon will continue to inspire generations of African surgeons for years to come.
May his soul rest in eternal peace.

❤የደም ስር መዘጋት እና “ጋንግሪን”❤️
o በሰውነታችን ውስጥ የተጣራ ደም ከልብ ወደ ልዩ ልዩ የሰውነት ክፍሎች የሚያሰራጩ "ደም ቀጂ /አከፋፋይ " ደም ስሮች(arteries) እና ጥቅም ላይ የዋለ ደምን ይጣራ ዘንድ ወደ ሳንባ እና ልብ የሚመልሱ "ደም-መልስ" ደም ስሮች(veins) ይገኛሉ።
o ዋና ዋና ደም አከፋፋይ የደም ስሮች (arteries) ሲዘጉ ፤ በደም ስሩ ምግብና ጠቃሚ ንጥረ ነገሮች ያገኝ የነበረው የሰውነት ክፍል (እጅ፣እግር፣አንጀት ወዘተ...) ሊሞት ይችላል፤ ይሄኔ "ጋንግሪን/gangrene" ሆነ ይባላል።
o በእርግጥ እንደተዘጋው የደም ስር አይነት እና ቦታ ታካሚዎች በ“ስትሮክ”፣ ልብ ድካም፣ በአንጀት መዘጋት ወይም በሌሎች ምልክቶች ወደ ህክምና ተቋም ሊመጡ ይችላሉ።
o የዚህ ፅሁፍ ዓላማ በተለይ በእጅና፡ እግር ላይ የሚከሰት አጣዳፊ የደም ስር መዘጋት በሽታ ምልክቶች እና ህክምናን በተመለከተ በወፍ በረር ለማስተዋወቅ ነው።
o የደም ስሮች በልዩ ልዩ ምክንያት ፤በአጣዳፊነት ወይም ቀስ በቀስ ሊዘጉ ይችላሉ።
ከአጣዳፊ ምክንያቶች መካከል : በድንገተኛ አደጋ(በመኪና፣ በጥይት ፣በጩቤ፣በማሽን አደጋ ወዘተ...) ፣በደም መርጋት ፣ከልብ በሚበተን የረጋ ደም(embolus)፤ ተጠቃሽ ናቸው። ይህንን በመሰለው ችግር ምክንያት የሚመጣ የደም ስር መዘጋት ፣ በአብዛኛው ፋታ የማይሰጥ ነው ። በመሆኑም በአስቸኳይ የደም ስር ህክምና/ቀዶ ህክምና ካልተደረገ በሰዓታት ውስጥ ደም ያጠረው የሰውነት አካል(እጅ፣እግር፣አንጀት ወዘተ...) ወደ መሞት("ጋንግሪን") ሊያመራ ይችላል ።
o ቀስ በቀስ የሚከሰት የደም ስር መጥበብ ቀድሞ የነበራቸው ታካሚዎች ሲሆኑ ከአጣዳፊው ክስተት በፊት ሌሎች ምልክቶች ይኖራቸዋል።ለምሳሌ በሚራመዱበት ጊዜ እግር ህመም (በአብዛኛው ባት እና ታፋ ላይ ) ይሰማል፣ ቆዳ ይደርቃል፣ ጥፍሮች ይጠነክራሉ፣ ወዘተ …ችግሩ እየተባባሰ ሄዶ አስከፊ ደረጃ ላይ ሲደርስ ያለምንም እንቅስቃሴ በተለይም በመኝታ ግዜ የእግር መርገጫ ና ጣቶችን ማቃጠል፣መቁሰልና መሰል ችግሮች ይኖራሉ።
አጣዳፊ የደም ስር መዘጋት ምልክቶች
የእጅ/እግር ደም ስሮች መዘጋት ልዩ ልዩ ምልክቶች ሊያሳይ ይችላል፡ ከነዚህም መካከል :
o የጣቶች ህመም/ማቃጠል፣ መደንዘዝ ፣ ውሃ መቋጠር ጣቶች፣እግርን/ እጅን ማዘዝ (ማንቀሳቀስ) አለመቻል;የእጅ እና እግር መጥቆር እና ፈሳሽ ማምጣት ሊሆን ይችላል
o የእግር የደም ስሮች ሆድ ውስጥ ካለው ትልቀ የደም ስር(aorta)ጀምሮ ድንገት ሲዘጉ የሁለቱም እግሮች አለመታዘዝ ሊያጋጥም ይችላል ::
ህክምናው እንዴት ነው?
· የደም ስር በአጣዳፊነት ተዘግቶ ባለበት ሁኔታ የምትጠፋ እያንዳንዷ ሰዓት የእጅና የእግርን የመትረፍ ዕድል እያመነመነው ይሄዳል(ህክምና ቢሰጥ እንኳ)። አልፎ ተርፎም በህይወትም ላይ ጭምር አደጋ ሊያመጣ ይችላል።
· ታካሚዎች በጊዜ ወደ ህክምና ተቋም ከመጡ - እንደበሽታው መንስኤ የተለያዩ አካል ማዳንን ታሳቢ ያደርጉ ህክምናዎች ሊሠጡ ይችላል:
· መድሃኒት በመስጠት (ደም ማቅጠኛዎችን ጨምሮ እንደ ስኳርና የደም ብዛት ላሉ ሌሎች ተጓዳኝ በሽታዎች)፣ እንዲሁም በቀዶ ህክምና የረጋ ደምን መጥረግ(thromoboembolectomy)፣
o ከአጣዳፊው የደም ስር መዘጋት/ደም መርጋት በፊት የነበረ(የከረመ)ለአጭር ርቀት የተዘጋ/የጠበበ ደም ስርን መጠገን/መለጠፍ(endarterectomy) ሊደርግ ይችላል።
o ከአጣዳፊው የደም ስር መዘጋት/ደም መርጋት በፊት የነበረ(የከረመ)ረጅም የሆነ የደም ሥር መጥበብ/መዘጋ ት በሚያጋጥምበት ግዜ bypass መሥራት ይቻላል
