A 45-year-old man presents with a two-year history of pain and swelling in the small joints of his hands and feet. He reports increased stiffness in these particular joints in the morning, which lasts for up to 2 hours.

He also complains of feeling generally tired and has lost some weight. He was previously fit and well, and does not take any regular medications.

On examination, his vital signs are all within the normal range.
