‘Little evidence medication treatment for chronic pain works,' says review
Medication should be the last resort. I have seen over and over again in clinic that hands on treatment, effective exercise, but crucially psychological belief/intervention can really help. The problem is often money, this approach is more expensive than medication. Harnessing the brains power to over come and maybe even accept a degree of pain is important. Techniques involving CBT, NLP, visualisation, meditation if used correctly and regularly can be really useful and crucially less harmful than medication.

FOAM ROLLING MASTERCLASS
I don’t recommend foam rolling generally, because in most cases it does not respect the fascial orientations and compresses it. Fascia needs hydration, ie. lots of water to fulfil one of its functions of being slippery, to allow muscles to slip and slide over one another. Compressing fascia might feel good, but it prevents gliding movements between structures. The method here I’m told respects fibre orientation, give it a go…I’d love feed back.

Feet are our much neglected foundation, the function of which can affect our entire body…from back problems to jaw problems.

For foot health, which will benefit your entire body…walk bare foot as often as possible... make a point of it at home and in the garden. If you experience discomfort doing this, then try short periods, regularly. If this is uncomfortable, try when sitting…working at a desk or in front of the TV, slipping shoes off, rolling them on a small ball (tennis, golf, bouncy), picking up a tea towel or small items such as marbles with your toes, spreading them, wiggling, basically move them lots! Just keep those shoes off as much as possible.

The use of orthotics is a controversial issue. Used long term, the foot muscles don’t have to work as hard, meaning they can sometimes exacerbate the problem. Foot, ankle, lower leg and glute strengthening exercises are the best treatment, with mobilisation and pumping of the foot ankle and pelvis and if necessary, orthotic use.

Expose yourself to natural terrain ✔️
Most human-made surfaces are flat, smooth, hard, and unnatural. Many of us spend most of our waking hours on these artificial surfaces, often while wearing footwear as well.
🔑 Exposing yourself to natural, uneven, rough, varied terrain is a great way to stimulate your body and can provide a lot of benefits:
🦶 Start with taking your shoes off and going outside. On grass, a beach, a trail, on rocks etc.
You will notice that it is uncomfortable, and that you have to pay way more attention to how and where you are stepping.
✅ A few things this will do:
- Toughen your feet up (and you up).
- desensitize your feet
- challenge your balance and stability more
- make you more aware of how you are moving
- force you to buffer impact better, naturally react to changes in the surface below etc.
- mobilize your feet and ankles more, and naturally massage your feet.
Start to introduce natural terrain into your life...It feels very good once you get used to it.

Spring is in the air...honest! This is a great time to check in with ourselves and press reset. This week we're looking at hydration.

HYDRATION - I notice winter dehydration in the body tissues of lots of my patients at this time of year. Many of us just don't drink enough water in the winter, it accounts for 50-70% of our body weight. Water, particularly 'free' water that the body can use straight away without having to process, is vital for every cell, tissue and organ in your body. Musculoskeletally, it keeps our intervertebral discs nice and plump, lubricates and cushions joints and the nervous system and our fascia, which covers our muscles, allowing them to slip and slide nicely over one another freely.

HOW MUCH WATER?
We often hear we should be drinking 2 litres of water day, however there is no evidence base for this. The optimum amount is actually unclear, although the most recent research from The University of Aberdeen who have collaborated with scientists across the world is currently the best advice to follow. However it also depends on your age, activity levels, time of year, climate, type of diet, health issues and medications.

- Current UK recommendations according to The British Nutritiion Foundation are of 6-8 glasses or 1.2-2 litres of fluid per day, including water, coffee, tea, squash, milks.

- The European Food Safety Authority recommends (assuming 20% from food, 80% fluids
- Adult and adolescent females 14yrs plus need 2 litres
- Adult and adolescent males 14yrs plus need 2.5 litres
- Pregnant women 2.3 litres
- Lactating women 2.7 litres

- The International Olympic Committee recommends 1.2 - 1.5 litres of fluid are consumed for each kilo of body weight lost during sports or exercise session to restore losses. Electrolytes Sodium and Potassium and Carbohydrate are also needed if exercise lasts longer than 1 hr.

- Most up to date recommendations from University of Aberdeen recommend the average person will need 3.6 litres, of fluid more than half of which will come from food, which means the amount needed to drink is 1.3 - 1.8 litres per day. Older people will require less, while in hot climes, being pregnant or breastfeeding and greater physical activity will increase it.

In my experience approximately 60% of my patients barely drink enough fluid and so I recommend introducing another large glass of just water in the morning and afternoon. Despite the recommendations of pretty much any fluid apart from alcohol counting, it seems sensible to try and include a good amount 'free' water, that will be immediately available to cells. Hot water is a good way of doing this in the winter. Avoiding sugary drinks is key, they are linked to increased risk of type 2 diabetes, dental caries and increased BMI in children.
