Starting next week, our new Social Prescribing Drop- In Hub launches at Paxton Medical Group Claughton . Join our social prescribers Wed 19 November between 9am-12noon. They will be on hand to support our patients from across the PCN. Social Prescribers can help patients who are struggling with:

Family issues
Housing problems
Fuel poverty
Debt
Gambling
Social isolation

The drop in hub will be open every Wednesday from 19th November through to the end of January. No appointment necessary.

Starting on Wednesday 18th November and running every Wednesday through to the end of January, will be our new Drop In Social Prescribing Hub based at Paxton Medical Group Claughton.

Our Social Prescribers can help with things that affect your health and wellbeing, but that cannot be treated by doctors or medicines alone, such as:

Social isolation
Stress due to problems with debt
Benefits worries
Concerns regarding housing
Family issues
Unemployment

Our Social Prescribers are well connected with a host of local neighbourhood organisations and are there to help you when you need somewhere to turn. The new Social Prescribing Hub will be open to all patients across the including:

Hosted by our PCN Pharmacist and Health Coach, the event will provide practical advice, awareness, information and guidance to support you to get the best from your treatment.
