1. Start by completing the eligibility form on our website.
2. To help our team understand your medical history, we will ask for supporting documents from your GP or any specialists you have seen. If you are transferring from another clinic, previous clinic letters will also be helpful.
3. After that, we will guide you through a few admin steps such as uploading photo ID and signing a treatment agreement.
