❓ Why do we need to carry out surgical periodontal therapy? This is a photo that I took of a case that our senior Periodontist Resident (and now Melbourne-based Specialist Periodontist) .bhargava was treating. The crater-type defect is surrounding the distal aspect of tooth #46 (LR6). No matter how much you blindly (non surgical) debride those roots, it is very unlikely that a defect of this size and type will be refilled with bone without intervention.

I feel that it is very important we don't ignore defects like this. I think watching or observing or maintaining disease can be described as clinical negligent. As Specialist Periodontists we should maintain health and not disease.
I place plenty of implants but it is more satisfying when we save a natural tooth. Periodontal regeneration procedures are very predictable (if done well) as the studies by Cortellini et al, Sculean et al etc show.

Dental implants are not always a straightforward procedure. There are numerous times that there is insufficient bone height in the posterior maxilla (upper jaw), therefore making dental implant placement difficult. For these cases, a technique-sensitive procedure commonly known as "Sinus Lift" is needed. The surgeon will place a bone graft in the floor of the sinus after carefully lifting the sinus membrane. If you are a dentist and want to refer your cases, please send me a message in DM. We will look after your patient, carry out the sinus procedure and after a few reviews, we will discharge the patient back to your care, and provide guidance on the timing of the implant placement, if you prefer to place the implants yourself.

This lovely patient😊 was referred for implant placement in the upper left and right quadrants of the mouth. I placed 2 implants in the missing teeth areas and carried out an atraumatic internal sinus lift in the upper left first molar area.
For my lovely patients, years of experience blended with the latest research allows me to carry out safe and predictable evidence-based implant dentistry. Having had specialist training, and subsequently worked in the postgraduate teaching sector both nationally and internationally, I put a huge emphasis on predictable dentistry.
