Hello! My name is Sean, I'm a Physiotherapist specialised in musculoskeletal and sporting injuries.

People often pay for a Fitness Coach or a Personal Trainer who support them along their fitness journey, but traditionally physiotherapists don't offer this level of support with patients.

This bugs me, because injury rehabilitation/pain management are often exercise in the presence of an injury, and patients deserve the ongoing support.

I care about my patient's results. That is why I combine elements of Coaching with Physiotherapy. This enables me to thoroughly support people throughout the process, whilst also being readily available to answer questions and make modifications where necessary.

My take:

The most applicable warm-up to an activity or exercise that you are about to do is the exact movement with a progressive increase in load.

Think about it, warming up to a heavy bench press by using a lighter load will use your muscles and joints in the most comparable way to a bench press; because it is the same movement.

AND I know I'd rather use the time doing more working sets!

Don't compromise on your quality of care. Let me explain...

A recent patient told me about their experience with another therapy clinic. They were assessed which revealed that their dominant shoulder sits lower (this is often normal, we are not symmetrical), but they were told “you are out of alignment” and this needs “fixing”. Something I sadly hear way too much.

The “fix” was a quick massage and cupping treatment that provided some short-term effects, and then some very general exercises for “3 x 10 each day”. Afterwards, they wanted to contact the therapist to ask some questions and were told that they need to pay again.

The belief that their body was “broken” became a driver of their pain and they thought only the therapist can “fix” it. After sinking hundreds of pounds into their treatment, their shoulder always reverted, because this is a variance (and adaptation) within their anatomy and over time their pain worsened, they felt lost, confused, and taken for granted.

At Impetus, we like to do things a little differently to give you the highest quality of care and support we can.

After a thorough assessment, we identified their shoulder pain was likely due to tendinopathy (tendon injury, which does not want to be poked and prodded) following an out of routine sporting event (they did a little bit too much too soon). Their treatment consisted of thorough advice and education to explain what is happening and how to manage it, and a progressive personalised exercise plan that fits around them:

The rehab plan was sent via app which allows for easy tracking and data collection to monitor and review progress.

We then had regular check-ins each week to review how the process was going and to discuss any questions. Alongside this, they were reassured that they could contact us at any time if they had a question.

We had extensive discussions to explore beliefs and expectations, discuss myths and misconceptions and offer reassurance. We acknowledge that pain and injury are multifactorial, and that sometimes progress takes time.

We realise that we are treating a person and not just a body part. We use an evidence-based, person-centred holistic approach to treatment to help you short term AND long term.

We are about promoting independence, whilst supporting you along the way.

Some small and big wins from a few of those I've worked with over the last week or so (all anonymous and confidential).

As a physio, I like to address things long-term by empowering people to move well, move better, and with less (and without) pain.

Something I often tell people is that "Rome wasn't built in a day," and just like this, long-standing sustainable improvements with physiotherapy can often take some time.

Coaching under the guidance of a musculoskeletal and sports physiotherapist.

Sports massage has multiple purposes.

Pre-event massage assists in warming up the body. It is performed before training or to prepare the body both physically and mentally. During a pre-event sports massage a wide range of techniques are used to ready the muscles for exercise by increasing their blood supply, joint mobility and improving mental state in a bid to increase performance!

Post-event or maintenance massage is performed between bouts of training or vigorous exercise such as a competition. It exists to aid the recovery process by using a range of techniques to help prevent and reduce symptoms of delayed muscle onset soreness.

Delayed onset muscles soreness occurs after and intense exercise and is associated with inflammation, stiffness and muscle fatigue.

This is also a good opportunity as it may allow the physiotherapist to identify any potential injuries forming, catching them early before they escalate.

Pain is modifiable.

Passive treatments are great at this, but do not confuse feeling better with getting better.

Yes, I offer sports massage, but please do not consider this a viable alternative to a solid injury management plan.

I view passive treatments such as massage as adjuncts, which can make the process more tolerable.

But if you don't address the cause of your pain, it will mostly likely come back.

Play the long-term game once, rather than repeating the short-term game over and over again.

Rehabbing an injury with resistance bands is often like trying to put out a fire with a water gun:

The logic is there, but in reality, it just doesn't cut it.

I take injury rehab seriously. This is why I choose to deliver physiotherapy out of a gym environment:

We have the equipment and resistance necessary to stimulate adaptation. Adaptation to make you stronger, faster, resilient, and confident.

All so that you can keep doing the sports and activities that you love.
