🚨 Emergency Care Plans Are Failing Our Most Vulnerable
Here’s what happened just last week to help change that — with My Emergency Plan

I published two white papers.
I gave a talk to an international audience.
I got emotional doing it...

But I’m just getting started.

🎤 I was honoured to share My Emergency Plan with a global audience through Project ECHO and Hospice UK.

🗣 I was humbled by these words from Professor Max Watson (MBE), Director of Project ECHO at Hospice UK:

“He brings an energy and a focus to advance care planning which I find really refreshing and energising… I've known him as someone who is an enthusiast, who's got energy and who makes things happen, and in our world of cynicism, Matt has just been a real light for me.”

🎧 You can listen to the talk here (audio/video): https://open.spotify.com/episode/31qZLX9jXbiqIhYaLFaxoB?t=2580

⏱ Skip to 43:03 for my 10-minute segment if you’re short on time.

I choked up talking about the 2+ million vulnerable people in the UK still without meaningful protection during medical crises.

💬 Other brilliant speakers included Dr Matthew Doré, Prof Kate Flemming, Emma Winstanley, and Anita Hayes.

🧾 The White Papers

Exposing the Blind Spot – The Problem
✅ Live audit of 270 urgent care patients over 72 hours
🧓 71 met vulnerability criteria (care homes, dementia, housebound):
• ❌ 96% had no plan, or one too vague to use
• ⚠️ 0/71 met robust standards
• 🧠 In the dementia subgroup:
– 84% raised legal or ethical red flags
– ⅓ had no plan at all

🙏 Grateful to the independent reviewers:
Prof Erica Borgstrom (The Open University)
Assoc Prof Matthew Allsop (University of Leeds)
Dr Clive Graham (Medical Examiner Officer Lead)

Before the Next Crisis – The Solution
✅ 23 patients received expert-led planning
📉 72.5% of potential hospitalisations avoided
⚰️ 100% of recorded deaths occurred outside hospital, in comfortable settings
📞 Multiple 999 calls avoided — not through restriction, but through clarity
🧑‍⚕️ Praised by families, a care home leader, and an independent medical examiner officer

🙏 Foreword by Professor Max Watson

Dr Teresa Storr — a retired NHS consultant with 45 years of experience in palliative medicine — just shared this about My Emergency Plan:

“This is a much needed service, and after working alongside Dr Gloudeman as his Hospice Consultant and Medical Director, I know he will deliver this important work sensitively and skilfully.”

I’m so grateful for her support. Dr.Storr is a true leader in compassionate care — and her encouragement means the world.

Some memories never fade — they bloom. 🌸🌿🌹

After losing our grandmas, my wife and I started a simple tradition: we plant a tree for each one we love and miss.

The Betty Tree. The Flamingo Tree. A beautiful Rose Bush. 🌳❤️

Every time we walk past them, it feels like they're still with us and they are still growing alongside us.

Maybe it will inspire you to plant a living memory too.

What I've Built Quietly for 7 Years Could Change Healthcare for Everyone in the UK and Beyond

Not in a startup accelerator.
Not with seed funding.
Not on social media.

But in care homes.
In hospital wards.
During my GP surgeries.
On night shifts.
At the bedside of patients who were scared, in pain, or dying... when no one else was there.

Over the last 7 years, I’ve quietly built something I believe in.

Because I kept seeing the same things:

- Preventable hospital admissions

- Families, like mine, unsure what to do during an emergency

- Suffering that could have been avoided with better and proactive planning

So I created a system.

A powerful, proactive, personalised emergency health care plan led by first of its kind experts:

- One that’s already reducing hospital admissions by over 70%

- One that’s helping people pass away - not in hospital corridors around strangers, but at home, in hospices, or in care homes, with dignity around loved ones.

I started this because I lost someone close during medical school, and I swore I’d never become a doctor who just “got through a shift.”

Now I’m ready to scale.

To stop working in silence and take this where it belongs:

Into every care home, to every housebound patient, to individuals struggling with dementia, and eventually—every country.

But I can’t do that alone.

I’m looking for others who see the vision.

Like-minded individuals who want to build something that truly matters.

If this resonates with you, or you know someone I should speak to please message me directly.

This is one of my favourite photos — me and my grandpa, sitting by the water near his home in one of his most cherished spots.

He once told me something that stuck with me and now lives on through my work:

“It’s not for the faint of heart, getting older.”

I’ve shared that line with many of my patients. It often brings a smile… but also gently reminds them that while I may not truly know their experience, I care deeply about someone who may understand.

That sentiment helped shape My Emergency Plan, a service dedicated to making sure no vulnerable patient is left without a plan — and no family is left unsure in life’s hardest moments.

🚨 Why I Built “My Emergency Plan”

(and why I’m asking you to share this)

I've worked in the NHS for nearly a decade across 8 key specialties — from acute medicine to geriatrics and palliative care — I kept seeing the same thing:

Vulnerable patients — those in care homes, housebound, or living with dementia — being rushed into hospital needlessly, often against their wishes.

Patients are scared. Families are unprepared.

Just like my family was when we faced this during my time in medical school.

So I built something to change that. I refined it over a number of years. And now it has been launched to help protect those that need it most.

📄 My Emergency Plan is a doctor-guided service that creates clear, compassionate, and clinically robust emergency care plans — before the crisis hits. I believe in informed decision making and these plans provide robust expert guidance so patients, families and carers can make the most informed decision possible.

We’ve already seen promising results:
✅ 72.5% reduction in hospital admissions
✅ Every death occurred in a home or hospice — not in a hospital (where 40–50% usually occur)
✅ Families and staff feeling confident, not panicked

It’s built to support:
- Care homes
- NHS commissioners
- Patients and Families who want peace of mind

It’s designed to save heartbreak, stress, and patients from avoidable suffering.

Oh — and yes, it could also save the NHS billions by protecting patient wishes and avoiding unnecessary admissions.
