Following comments in the recent NHS Experience Survey, Preseli Practice is inviting patients to take part in a short questionnaire about Continuity of Care. Your views will help us review and improve our services.

Yn dilyn sylwadau yn Arolwg Profiad diweddar yr NHS, mae Practis Preseli yn gwahodd cleifion i gymryd rhan mewn holiadur byr am Parhad Gofal. Bydd eich barn yn ein helpu i adolygu a gwella ein gwasanaethau.

📝 Please take a few moments to complete the survey.
📝 Byddem yn gwerthfawrogi pe gallech dreulio ychydig funudau i gwblhau’r arolwg.

We want to hear from you on how you currently access community pharmacy services. Information and insights you provide will help us to update our Pharmaceutical Needs Assessment.

Our assessment looks at where pharmacies are located and how far patients have to travel, what services pharmacies offer as well as the services that patients access most and if current services meet the needs of our communities.

The survey closes on Monday 9 March. Complete the survey online at www.haveyoursay.hduhb.wales.nhs.uk, collect a paper copy from your local community pharmacy or request a copy by emailing communitypharmacy.hdd@wales.nhs.uk or calling 0300 303 8322.
