Patient Update:

Dear Patients,

We have previously contacted Healthwatch and Cat Smith MP reference the ongoing Knott End Pharmacy closures. We told Cat we have approximately 11,500 patients in our community and that we wanted to bring to her attention a pressing issue that has been significantly affecting our patients' access to necessary healthcare: the frequent unplanned closures of Jhoots Pharmacy, Knott End due to the unavailability of a pharmacist.

Over the past 12 months, we have observed a high frequency of unplanned closures at Jhoots Pharmacy, including the current closure.

Reliable pharmacy services are crucial for the health and well-being of our community. Which leads to the question: “When Knott End Pharmacy is closed, why can’t Over Wyre Medical Centre Dispensary issue the medication on my repeat prescription?” The reason we cannot is because there are strict rules governing our Dispensary with who we are allowed to dispense medication to. If patients live more than a mile away of a dispensing pharmacy, they are eligible to use the dispensing services at Over Wyre Medical Centre. Unfortunately, we are not permanently allowed to dispense to any who live within a mile of Knott End pharmacy.

In regards to the situation with J-Hoots Pharmacy Knott-end, Dr Holmes GP Partner & C Hill Practice Pharmacist engaged in a meeting with the Pharmacy Contracts team at the Integrated Care Board (ICB) to offer our support and shared frustrations with our community and requested a temporary suspension of the 1 mile rule in order for us to support our Knott-End patients. Unfortunately, the definition of the approved suspension of the 1-mile rule is for emergency situations only and not for the dispensing of routine repeat items.

Sadly, this means that we have received approval to dispense emergency medication for Knott-End patients but are not permitted to dispense repeat prescriptions for patients. Regrettably patients with repeat medication requests will need to continue to have their medication dispensed by Hambleton Pharmacy or alternative online chemist.

We would sincerely like to support our community and dispense repeat medication but despite our protestations, the powers that be have decided the only help we are permitted to provide is by dispensing emergency medication only.

I hope this explains the situation.

PSA Testing: What You Need to Know 🩺

Many patients ask about PSA (Prostate-Specific Antigen) blood tests for prostate problems. Here’s what the evidence tells us:

The Reality About PSA Testing:

PSA is NOT a perfect screening tool. Raised levels can occur due to:
• Benign prostate enlargement
• Urinary infections
• Recent ej*******on or cycling
• Some medications

Normal PSA doesn’t rule out cancer - about 15% of prostate cancers occur with normal PSA levels.

Why There’s No National Screening Programme:

The UK National Screening Committee reviewed the evidence and found:

• Overdiagnosis risk:

Many slow-growing cancers may never cause harm

• False positives:

Leading to unnecessary anxiety and invasive biopsies

• Treatment complications:

Surgery/radiotherapy can cause incontinence and erectile dysfunction

• Uncertain benefit:

Limited evidence that screening reduces death rates

Higher Risk Groups:

Some men have increased prostate cancer risk:
• Black men (2-3x higher risk, earlier onset)
• Family history of prostate/breast cancer (especially father/brother with prostate cancer, or BRCA gene mutations)
• Men aged 50+ (risk increases with age)

These groups may benefit from earlier discussions about PSA testing from age 45-50.

Should Asymptomatic Men Request PSA Testing?

This is where informed choice becomes crucial. For men without symptoms:
• The evidence for benefit is limited and uncertain
• Risk of overdiagnosis and unnecessary treatment is highest

• We’ll discuss your individual circumstances, including risk factors

If you have symptoms (difficulty urinating, frequent urination, blood in urine), PSA may be part of your clinical assessment.
