π 2025 RCUK Resus Guidelines: Whatβs New?
The Resuscitation Council UK has released its 2025 updates. Hereβs what every responder, clinician, and instructor should know:
π§ Education
β’ CPR now begins in schools from age 4β6, with annual refreshers
β’ Training should be role-specific and tech-enhanced (feedback devices, VR, hybrid models)
β’ Gamified and rapid-cycle learning encouraged
β’ Faculty development and βtrain-the-trainerβ programmes emphasised
π The Numbers
β’ ~115,000 out-of-hospital cardiac arrests (OHCA) per year in the UK
β’ ~43,000 receive attempted resuscitation
β’ Bystander CPR in 60β73% of cases
β’ Public AED use remains under 10%
β’ 30-day survival around 9β10%; higher (β30%) in the Utstein subgroup
β€οΈ Basic Life Support (BLS)
β’ Call 999 before checking for breathing
β’ Compression-only CPR acceptable for untrained rescuers
β’ 100β120 per minute, 5β6 cm depth, minimal interruptions
β’ Early defibrillation remains critical
π Advanced Life Support (ALS)
β’ No changes to drug intervals or doses
β’ Focus on ventilation quality and correct pad placement
β’ Team debriefs and performance feedback encouraged
β’ CPR-induced consciousness should be recognised and managed appropriately

β’ Person-centred discussions and advance planning highlighted
β’ Family presence should be offered where feasible and supported
β’ Decisions to stop resuscitation must consider reversible causes and patient values/wishes
β’ Ethical simulation and debriefing recommended to build decision-making confidence
π Systems Saving Lives
β’ Strengthened first responder and AED networks
β’ Mandatory CPR education for schoolchildren and learner drivers
β’ Support for lay rescuers after the event encouraged
β’ Continuous data-driven system improvement promoted
πΆ Paediatrics & Newborns
β’ Assess heart rate, breathing, and tone β not skin colour
β’ Adrenaline every 4 minutes (no interval change)
β’ Sodium bicarbonate removed from newborn protocols
β’ Out-of-hospital births (including ambulance settings) now explicitly covered in NLS guidance
β’ Early use of supraglottic airways and two-person airway management encouraged
π§ Post-Resuscitation Care
β’ Maintain systolic BP > 100 mmHg (MAP β₯ 60β65)
β’ Prevent fever (β€ 37.5 Β°C) for 36β72 hours
β’ Treat seizures with levetiracetam or valproate
β’ Prognosticate β₯ 72 hours post-ROSC using multimodal assessment
β’ Emphasise early rehab, psychological support, and family follow-up
π¨ First Aid β Brand New Chapter
β’ First aid is officially recognised as the first link in the chain of survival
β’ Includes choking, severe bleeding, anaphylaxis, asthma, overdose, heat illness, hypothermia, and more
β’ Accessible, inclusive training promoted across communities, schools, workplaces, and remote environments
