A tongue tie update? Barbara and Nancy discuss a 2026 research study on tongue ties by Raol et al. and a commentary response in this episode of All Things Breastfeeding. One of the goals of LactaLearning is to provide recent studies that have the potential to impact clinical lactation practices. The debate over whether tongue ties are being over- or under-treated has been ongoing for several years. After reviewing the latest research on tongue ties for the upcoming edition of Breastfeeding Answers (this edition is still at least a year away from being released), the research conclusion seems to be that there are absolutely cases where a tongue tie release appeared to be critical for an infant to be able to nurse effectively and/or without pain for the parent. On the other hand, it appears that the number of babies undergoing this procedure is increasing, even though tongue tie may not have been the primary issue.

I've been given permission to share this incredible video from a mum who is pumping breastmilk for her baby. This is a great example showing the fat content of breastmilk. Fat is the second largest component of human milk, it has an important role in energy content and the development of baby's central nervous system. Fats in human milk are designed to be easily absorbed and complement enzymes in the milk which help to break them down for digestion.

Happy IBCLC day to all my incredible colleagues.
Its a privilege to be able to support families and a real honour to be part of a feeding journey, whether its at a peer supporter led group, workshop or one to one appointment.

At today's baby massage and feeding support course we discussed cluster feeding.
When baby is feeding frequently, especially in the evenings, its understandable to feel like baby isn't getting 'enough' milk or worries about how baby can still be hungry and if you have milk to give.
We discussed that the breast works more like a factory than a storage unit, it's always in production and it will change the breast milk composition according to your baby's needs. Its likely in cluster feeding that the calories in breastmilk increase, fussiness is often due to slightly slower flow rather than volume.
Top tips to help include, switching sides, using breast compressions to increase flow, being relaxed and making sure you have lots of snacks and drinks to keep you fed and hydrated during this time.
