Why treating the lower face first can make you look older.
Most people think jowls or lower face heaviness start in the jawline.
In reality, facial ageing often begins higher in the face due to gradual changes in bone structure, fat pads, collagen levels and skin elasticity.
A youthful face naturally forms what is known as the Triangle of Youth, where volume is concentrated through the cheeks and mid-face and the lower face appears lighter and more defined.
As structural support gradually reduces with age, this triangle slowly reverses. Volume can shift downward, contributing to changes such as flatter cheeks, deeper folds and lower face heaviness.
This is why treating the lower face first doesn’t always create the most natural result. If filler is added to an area that already appears heavier, it can sometimes make the lower face look even fuller.
By restoring support in the mid-face first, the face can often rebalance naturally. Supporting the cheeks helps lift and reposition soft tissue, which may soften the lower face without needing as much filler — and sometimes no filler at all in the lower face.
When I assess a patient’s face, I focus on facial structure and balance, rather than simply treating individual lines.
