As we come to the end of 2025, as a PCN we have had a really positive year in terms of community support and engagement. We have successfully delivered 12 patient events to over 740 patients , delivered our first networking event (with over 50 organisations attending), we have set up 2 new social groups (Sewing Social Group and a Diabetes Support Group), delivered 35 workshops to over 260 patients across various community venues, supported 10 patients to take up a hobby aiming to improve their MH (our thanks to The little Push Project), delivered over 25 Winter Warm Packs (thanks to Green Doctor West Midlands)and referred in several patients for Christmas meals (thanks to SwanBank). None of our achievements mentioned above could have taken place without the support of our various partners that we continue to work with for the benefit of our patients, and our continual support provided by our PCN Staff that help to make us a great team. So for this, we thank you.
Next year we are excited to be looking to build on the above. We have already sent out the 2026 calendar of PCN events, we will also be looking at starting new groups, delivering new workshops and driving more opportunities to work collaboratively across the city for the benefit of our members and community.
Finally, we would like to wish all our patients, partners and support services a peaceful and restful Christmas and happy and healthy New Year.
