Honored to attend the 4th InaSAU Meeting 2025, a scientific forum dedicated to comprehensive men’s health.

On this occasion, I had the opportunity to share insights on the role of Transurethral Resection of the Ejaculatory Ducts (TURED) in managing ejaculatory duct obstruction (EDO)—a correctable yet often under-recognized cause of male infertility.

Clinical application of TURED:
• Indicated in patients with confirmed EDO (low ej*****te volume, azoos***mia/oligozoos***mia, dilated seminal vesicles, or midline cysts)
• Endoscopic approach allowing direct relief of distal seminal tract obstruction
• Can be performed as a minimally invasive, fertility-preserving procedure

Key benefits of TURED:
• Restoration of antegrade ejaculatory flow
• Improvement in semen volume, s***m concentration, and motility
• Potential to achieve natural conception or improve outcomes of assisted reproduction
• Avoids more invasive s***m retrieval procedures in selected cases

A reminder that targeted diagnosis and appropriate intervention can significantly change reproductive outcomes in men with obstructive pathology.

Endoscopic Combined Intrarenal Surgery (ECIRS)
Two approaches. One goal. Better stone clearance.

ECIRS combines simultaneous retrograde intrarenal surgery (RIRS) and percutaneous nephrolithotomy (PCNL) to optimize stone clearance, particularly in complex, large, or staghorn renal stones.

Blue Laser Vaporization of the Prostate (BLV-P) — Post-Operative Evaluation

Proud to share a case of Blue Laser Vaporization of the Prostate with an excellent early functional and anatomic outcome (1 month). The blue laser’s shallow pe*******on depth and strong coagulative effect allow precise tissue ablation with minimal bleeding, making it a compelling option for selected BPH patients—especially those prioritizing fast recovery and functional preservation.

Post-op evaluation (early):
• Uroflowmetry: Qmax 27.3 mL/s, Qavg 18.6 mL/s
• Voided volume: 242 mL
• PVR: 44 mL
• IPSS: 7 (mild symptoms)
• QoL score: 1
• Imaging: Well-opened prostatic fossa on ultrasound/endoscopic view

These results reflect significant relief of obstruction, good flow dynamics, and high patient satisfaction shortly after surgery.

Blue Laser Vaporization of the Prostate
A new, minimally invasive option for Benign Prostate Enlargement (BPE)

Recently we performed Blue Laser Vaporization of the Prostate, an advanced technique that offers excellent tissue removal with minimal blood loss thanks to its unique laser wavelength and strong coagulation zone. This makes the procedure safer and cleaner, even in vascular prostates.

A Challenging Kidney Stone Case — with a Happy Ending

Sometimes, what looks small on the outside can be massive on the inside.
This patient came with a large “staghorn” kidney stone — measuring 6.4 cm — filling almost the entire right kidney. It caused swelling (moderate hydronephrosis) and required a special minimally invasive procedure called PCNL (Percutaneous Nephrolithotomy) to remove it.

The left kidney also had a few simple cysts, but thankfully they were harmless (Bosniak I).

💪 After a precise and carefully planned surgery, the patient was completely stone-free and was discharged the very next day — pain-free and recovering well.

Cases like this remind us how important it is to stay hydrated, check your kidneys regularly, and treat stones early before they grow too large.

Our Initial Experience with Local Anesthesia Prone PCNL in .ui

We present our first case of a 71-year-old male patient with a history of congestive heart failure post-PCI. He had to stop his blood thinners for 1 week before surgery. With an ejection fraction of only 13%, both spinal and general anesthesia were deemed too risky.

Otherwise active and healthy, we proceeded with local anesthesia. An ultrasound-guided flank block was performed, and surgery continued with a standard PCNL using a 30 Fr Amplatz sheath, dilated with Alken metal dilators.

This case demonstrates that in carefully selected patients, prone PCNL under local anesthesia can be a safe and feasible alternative when general or spinal anesthesia are contraindicated.
