As part of the post surgery rehabilitation of a total hip arthroplasty a set of precautions are given to enable risk free recovery. The delivery of these precautions to the patient is extremely important. If the reason for this caution is not explained fully there is the potential for fear of movement, avoidance of exercise and decreased mobility all of which are essential for recovery.
The fear stems from a thought in the back of the mind that if I go too far the new joint will dislocate and that thought is frightening. However, looking at statistics the risk of dislocation of primary hip replacements ranges from 0.2% to 10% per year.
This is a major surgery with a large amount of tissue repair required in the first 6-8 weeks. The precautions, when followed alongside a rehabilitation programme allow the tissue to heal, adapt and prepare for the increasing load on the road to recovery.

Post surgery I was unable to sleep due to the intense levels of pain. I couldn’t get comfortable in bed due to post surgery precautions and healing surgical sites. I looked at my Garmin one morning and the stress readings on the device clearly showed the night I had had.

The Physiological Effects of Acute Pain

Acute pain is our body’s immediate response to injury or illness, serving as a protective mechanism. But did you know it triggers a series of physiological changes? Here’s what happens when acute pain kicks in:

1. 🌀 Fight or Flight Activation: The body’s stress response, or “fight or flight,” is activated. This releases adrenaline and cortisol, which increases heart rate and blood pressure, preparing the body to react.
2. 💔 Increased Heart Rate and Blood Pressure: As the nervous system ramps up, your heart pumps faster to supply more oxygen and nutrients to essential organs and muscles.
3. 🌬️ Rapid Breathing: Pain can cause you to breathe faster or more shallowly as your body tries to bring in more oxygen. This can sometimes lead to dizziness or hyperventilation.

6. ⚡ Heightened Sensitivity: The nervous system becomes more sensitive to stimuli, increasing the perception of pain as a way to warn against further harm.

From the moment I made the decision to start JB Physiotherapy and move from Corbally to establish alongside my goal has been simple.
I wanted to provide an honest, complete Physiotherapy approach to each patient coming in my door. I have focused my own career on a treatment approach putting the person first to aim to increase their quality of life. I have worked with the general and sporting population as well as working as a clinical tutor and I continue to have a love for my profession. I have been lucky to work alongside inter-county athletes, an Olympic athlete and currently two excellent practitioners who bring so much to the care and service we offer.
We offer a complete assessment process to guide the treatment and rehabilitation process. Making use of and we have a detailed approach to return to function and return to play. David is offering pre-season assessments using our objective measurement tools and our strength and conditioning facility. He also provides in-depth orthopaedic treatment and rehabilitation plans as well all sport related injuries. Aoife is currently offering deep tissue massage as well supervision and guidance of our rehabilitation blocks.
We are excited about where the practice currently is and the plans for the coming year. With that in mind we would be delighted to make our services available to all who seek Physiotherapy care.

Sports massage now available with Aoife. Aoife is a 2022 graduate of the University of Limerick where she completed her Bachelor of Science Honours Degree in Sport & Exercise Science. She simultaneously completed a diploma in Deep Tissue Massage/Sports Massage while finishing her degree this year and also holds a second diploma in Sport Psychology. She will be furthering her academic achievements next year when she starts her masters of Physiotherapy.

As part of her academic studies, Aoife has completed clinical placements in private settings involving both acute care and prolonged rehabilitation care. She has experience working with youth and adult athletes as well as working with underage development teams in a coaching role and as a sports masseuse. In addition to this, she has experience with post-operative patients, with neurological and respiratory conditions, musculoskeletal disorders and the elderly population. Through her studies and various placements, Aoife has seen the benefit of physical activity & exercise on clients from all populations and backgrounds. This, along with manual therapy, is what drives her to be passionate about her job.

Aoife has additional training in First Aid, Sports First Aid and CPR Training.
She herself has been involved in sports all her life, playing both camogie and soccer with clubs here in Limerick.

Working in patient care, Aoife believes in an empathetic and caring treatment process helping her clients to regain and maintain a good quality of life while always promoting the health and well being of all the clients she meets.

We are delighted to welcome David Rowsome to our Physiotherapy team here at JB Physiotherapy 👊🏻

David graduated from the University of Limerick in 2019 with a BSc in Physiotherapy. Since graduation he has been working in an acute hospital setting and also in several sporting settings. David is a CORU registered physiotherapist and a member of the Irish Society of Chartered Physiotherapists (MISCP).

Since graduating, David has worked in an acute hospital, working in areas such as orthopaedics, seeing patients after total hip replacements, total knee replacements, ACL surgery, and shoulder surgery, to seeing critically ill patients in the ICU, and rehabilitating them. He has also worked in the areas of oncology and care of the elderly, helping people of a wide variety of backgrounds. David has supervised programmes in Cardiac Rehabilitation and Living with Parkinson’s Disease.

In sporting setups, David has worked with UL Bohemians RFC, South Liberties Senior Men’s Hurling, and currently the Limerick Senior Men’s Football. He plays rugby himself in the All-Ireland League with UL Bohs.

Throughout this experience David has developed a love for rehabilitation of people from any population, and has seen first hand the impact of exercise in getting people back to their normal function. He has a massive interest in strength and conditioning and its importance in the general population, from patients with back or neck pain or after joint replacement, to athletes after surgery. David is also Trigger Point Dry Needling certified, and has used dry needling with a variety of patients.

He believes in a holistic, patient-centered focus in his treatment of all patients of any background of all ages. His love of rehabilitation and helping patients achieve their goals is what drives him as a physiotherapist.

In March 2020, just prior to the onset of a medical #@!” storm in conjunction with welcomed to Limerick to host his 2 day course.

The 2 days were highly informative, non-complicated, practically focused and great fun.

For me I was blown away blown away at how humble and helpful Adam was. He shared his knowledge openly and in a no BS manner helped increase my own ability to approach shoulder cases.

We are delighted to be welcoming Adam back to Limerick this September 17th and 18th.

The course is open to Physiotherapists, Physical Therapists, Strength & Conditioning coaches, and those work with clients that require their practitioners to have a complete ability to address shoulder issues.

We promise you will not regret attending this course and you will be a better practitioner after 2 days learning from Adam.

The course is eligible for CPD, it will give you a chance to meet colleagues from outside your own work environment, and give you a chance to learn from the forward thinking

🟢 Anterior Cruciate Ligament Rehabilitation 🟢

A well researched and discussed long term injury is that of the ACL. Whether the injury comes about as a result of contact or non-contact, the post surgical rehabilitation process requires up to 12 months of dedication and commitment to the outcome. In line with the direction and guidance of the attending consultant, design and implementation of the rehabilitation process needs to be goal orientated and pre-determined results driven.

Each phase needs to be designed with specific goals in mind from:
➡️ range of motion
➡️ bilateral strength comparison
➡️ force absorption
➡️ velocity training
➡️ return to contact

In our rehabilitation facility, we have the ability to implement this process. The use of enables the objective measurement of the process.

I’ve been working with DJ and for over a decade and we’ve had many a discussion about research projects. This week we put the wheels in motion for a study which commences this September.
Using .it and we will be conducting a number of tests to assess performance across the season with O’Connor Cup squad. The results of this will form the background for a follow on 4-year study looking at injury occurrence through a female players college career.
We had the pleasure of putting things in motion with in state of the art PESS building. It’s an exciting time ahead and the facilities and technology present in will enable the building of future research studies.
