Today is the day!

Today our Chartered Physiotherapist will be hiking the 4 highest peaks in Ireland with 4 others.

This consists of 12hrs 15mins of hiking and 14hrs 32mins of driving (3hrs 26mins of this is to get to the startpoint!)

We have currently raised a whopping €37,900!! Our aim is to get to €40,000!

🙌🏼 Delighted to announce that our Chartered Physiotherapist will be completing this mammoth hike and volunteering her physio expertise to help , and friends get over the line for this amazing cause.

🥾 They will climb the highest peak in each province in Ireland all within a 24 hour period.

The peaks include:
🏔️ Mweelrea, Co. Mayo
🏔️ Carrauntoohill, Co. Kerry
🏔️ Lugnaquilla, Co. Wicklow
🏔️ Slieve Donard, Co. Down

⚠️ Total elevation = 3627m

👶🏼 As you will see when you click the link, their inspiration behind this fundraiser is Colm and Valerie’s amazing daughter Indie and her journey since birth.

❓ Have you been told your “Sacroiliac Joint is out of place”? (SI Joint)

⚠️ Have you attended hands on treatment once to twice per week for 12 weeks to help solve your problem?

🚶‍♀️ It looks like you have been taken for a walk my friend!

📚 This study looked at the “Effects of Mobilization Treatment on Sacroiliac Joint Dysfunction” and the key points were:
• Exercise, with or without the addition of manual therapy, may help people with sacroiliac pain.
• The effectiveness of manual therapy alone in the treatment of sacroiliac dysfunction has not been established.

📈 Sacroiliac joint dysfunction syndrome (SIJDS) may be present in 15% to 30% of people with low back pain.

📕 Methods in this study:
64 participants
• One Group assigned SIJ Home Exercise Programme
• One Group received SIJ Manual Therapy and the same SIJ home exercise program.

🏋🏽‍♀️ Home Exercise Programme consisted of a stretching and strengthening 5 times twice per day for 3 weeks. The exercises were as follows:
• Quadriceps Stretch
• Adductor Stretch
• Piriformis Stretch
• Single Knee to Chest
• Both Knees to Chest
• Hamstring Stretch
• Lower Lumbar Spine Rotations
• Isometric Hip Adduction
• Isometric Hip Abduction

⚱️ Despite the lack of a gold standard to diagnose SIJD, clinicians still aim to help people suffering from pain in the SIJ region.

📖 This study found no significant difference between manual therapy and exercise when compared to exercise alone for people suffering from sacroiliac joint dysfunction syndrome.

🏃🏻‍♀️ This falls in line with current practice guidelines regarding treatment for diagnoses such as low back pain. Advice
to keep moving, avoid bed rest, and perform general exercise are all recommended for non- specific low back pain.

⁉️Can the anterior cruciate ligament (ACL) heal?

❗️Key Points
1. There is exploratory evidence that there may be signs of ACL healing at 2 and 5 yrs after injury.

2. Participants with evidence of ACL healing appeared to have better Outcomes at 2 years, but not 5 yr follow ups, when compared to nonhealed, early, or delayed ACL replacement groups.

3. Spontaneous ACL healing may occur more frequently than previously thought.

🦵🏼2 Groups
Group 1: 62 - Early ACL Reconstruction (ACLR) (Surgery)
Group 2: 59 - Rehab with the option of delayed surgery (51% of patients in this delayed group had ACLR within 5 yrs.)

📖 Results
2-yr follow-up: 30% of participants in Group 2 had evidence of ACL healing on MRI. Of these, 24 /54 participants went on to have surgery.

Of the 30 remaining in the rehabilitation only group (Group 2), 53% had evidence of ACL healing at 2 yrs. At 5 yrs, 58% had evidence of ACL healing.

Participants with evidence of ACL healing reported better outcomes at 2 yrs over participants with non-healed ACL or those in the early or delayed ACLR Surgical groups.

However, at 5 yrs, all groups had similar outcomes.

⚠️Limitations
This study was a secondary analysis and was not powered (i.e. sufficient sample size) to answer the research question. The small sample size and wide confidence intervals indicates a lack of certainty in the conclusions and findings should be interpreted with caution.

This study was conducted in recreational athletes, and we are unable to generalize to elite athletes or patients in jobs that place high demand on their knees (e.g. firefighters).

🏋🏽‍♀️ Clinical Implications

While this study provides exploratory evidence that the ACL can heal spontaneously, it by no means replaces our current knowledge and management of ACL tears. However, it does shed light on the fact that patients will benefit from a period of rehabilitation first before deciding upon surgical or non-surgical management, and perhaps one of these benefits is potential healing of the ACL.

We still do not know who or what type of patients may lead to ACL healing and current trials are underway regarding bracing acute ACL tears to promote healing.
