🔎 Case of the Week – Fetal Arrhythmia

This week, we encountered a fascinating case during fetal echocardiography: Second-degree Atrioventricular (AV) Block with 3:1 conduction.

🫀 What is it?
Second-degree AV block occurs when atrial impulses intermittently fail to conduct to the ventricles. In fetuses, this can be picked up using M-mode or Doppler echocardiography, where atrial (A) and ventricular (V) contractions are tracked.

⚡ Types in Fetuses :

Mobitz I (Wenckebach) : Progressive PR interval prolongation until a ventricular beat is dropped. Rare and often benign.

Mobitz II : Sudden non-conducted atrial beats without PR prolongation. More concerning, often linked to maternal autoimmune antibodies (Anti-Ro/SSA, Anti-La/SSB), with risk of progression to complete heart block.

👩‍⚕️ Why it matters :
Early detection is critical. While Mobitz I may be transient, Mobitz II needs close surveillance and sometimes maternal therapy to prevent adverse outcomes like fetal hydrops or complete AV block.

💡 This case highlights the importance of meticulous fetal cardiac evaluation whenever an irregular rhythm is suspected on antenatal scans.

Case of the Week - Congenital Ranula

A 21 weeker antenatal patient came for her routine Level-2 Anomaly scan.

Scan findings revealed -

1. Well defined cyst in the submandibular region.
2. Follow up scan revealed no changes in the size or the position of the cyst.
3. Post delivery the patient was diagnosed with a Ranula and was operated for the same.

A 12 weeker antenatal lady came to us for her routine NT-NB/ Level 1 Anomaly scan.
The Fetus showed Bladder extrophy with Omphalocele. The fetal spine also showed a kyphoscoliotic deformity.

Findings were suggestive of OEIS Complex (Omphalocele, Bladder Extrophy, Imperforate A**s & Spinal Anomaly).

Case of the Week

A young 30 year old girl came to me with redness in right Breast. The patient had no fever or pain. Her physician had given her a course of antibiotics for last 10 days with no improvement.

On USG, diffuse skin thickening was seen and axillary lymph nodes were enlarged with loss of fatty hilum. Though these findings can be seen in mastitis, but no response to antibiotics and a normal CBC were a bit concerning. We did a bilateral Mammogram which showed diffuse skin thickening and no obvious mass.

A punch biopsy from skin came out to be Inflammatory Breast CA (ER- PR- HER-2+).
The patient is currently undergoing Neo-Adjuvant chemotherapy.

I am delighted to share that I have recently joined as Director & Consultant Radiologist at Dr. Dua's Diagnostics & Imaging Centre.

This venture is my own private practice at SCO-16, Sector-16, Chandigarh.

Our Centre is fully equipped with -->

📍 All Fetal Medicine & Prenatal Diagnostics (NT/NB, Level-2 scan, Fetal Echo, Amniocentesis, CVS, Genetic Testing & Counselling).

📍 Breast Ultrasounds & Ultrasound guided Interventions (including Breast Biopsies, FNACs, Marker Placement & Wire Guided Localizations).

📍We shall soon be installing a State of the Art Japanese Digital Mammography machine by FUJIFILM by this month end which shall be the First Installation in any diagnostic centre in Punjab and Chandigarh Tricity region. It has a patented technology for minimum discomfort to the patient and low radiation dose.

📍We are also doing all General Ultrasounds, Colour Dopplers, Elastographies, Liver Fat quantification, USG Fibroscan & Lab Tests.
