MEDICAL THERAPY FOR GASTROPARESIS:

☑️ Domperidone is a peripheral dopamine antagonist that does👉not cross the blood–brain barrier.

✔️ The risk of extrapyramidal👉symptoms is minimal.

✔️There is an advisory for ventricular arrhythmia and sudden 👉cardiac death, yet the cardiac side effect👉profile, especially at👉 doses of 30 mg daily or lower, seems favorable👉compared with that of 👉metoclopramide

⏫ In December 2024👉 the FDA issued a letter warning that the current👉supply of domperidone under the expanded access IND could beexhausted in early 2025👉 efforts to find a new supplier are underway.

🟤 All patients on metoclopramide and domperidone👉should have regular electrocardiograms and electrolyte monitoring and check-ins to👉 monitor for development of extrapyramidal symptoms.

🔴 One notable prokinetic is prucalopride, which is FDAapproved for chronic idiopathic constipation
