🦴 Chronic MCP Joint Dislocation? The Volar Plate Might Be the Culprit

Chronic dislocations of the metacarpophalangeal (MCP) joint are uncommon but can be frustrating for both patients and providers. One often overlooked cause? Volar plate obstruction.

In longstanding dislocations, the volar plate can become interposed within the joint space, mechanically preventing reduction. This is especially relevant in neglected or misdiagnosed cases, where soft tissue adapts over time and fibrosis may occur.

🔍 Clinical Pearls:
• Chronic MCP dislocation may present with limited ROM, joint deformity, or pain.
• Attempted closed reductions may fail due to soft tissue entrapment.
• Imaging (especially MRI) can help identify interposed structures like the volar plate.
• Surgical intervention is often required to clear the obstruction and stabilize the joint.

💡 Early recognition and appropriate referral are key to preserving hand function.
