Today, let’s talk about what’s new in the 2025 KDIGO guidelines for managing Nephrotic Syndrome in Children.

🧠 1. Understanding the Update
KDIGO – or Kidney Disease: Improving Global Outcomes – released its new guideline for childhood nephrotic syndrome in 2025.
The focus this time is on personalized care, reducing steroid side-effects, and using genetic and biopsy tools wisely.

💉 2. Steroid Use – Shorter and Smarter
Earlier, we often used long courses of steroids for the first episode.
Now, the 2025 KDIGO update supports shorter initial steroid courses – around 8–12 weeks instead of the very long schedules used before.
Studies show this achieves the same remission rates with fewer side-effects like weight gain, mood changes, and growth issues.
So – shorter course, same benefit, fewer problems.

🧬 3. Biopsy and Genetic Testing – Clearer Pathways
The new guideline introduces a stepwise algorithm to decide when to do a kidney biopsy or order genetic testing.
This helps identify children whose disease may not respond to steroids or who have a genetic form of nephrotic syndrome.
In practical terms – it means earlier, smarter diagnosis and avoiding unnecessary treatments.

💊 4. Immunosuppressive Agents – No Fixed Order
Previously, we followed a “first-line” and “second-line” list of medicines like cyclophosphamide, MMF, tacrolimus, or rituximab.
The 2025 KDIGO guideline now says — there is no strict hierarchy.
The best medicine should be chosen based on:

your child’s pattern of relapses,

side-effect profile,

cost and availability, and

family preference.
This approach encourages shared decision-making between parents and doctors.

🌍 5. Aligning Definitions and Global Relevance
KDIGO 2025 harmonizes definitions — such as frequent relapses, steroid-dependence, and steroid-resistance — with other international societies.
This means better global consistency and easier comparison of studies.
For countries like India, it helps integrate evidence-based care while respecting local realities.

⚖️ 6. Focus on Reducing Treatment Burden
A big theme is minimising toxicity and focusing on the child’s growth, nutrition, and mental well-being.
Doctors are encouraged to avoid overtreatment and tailor the plan to each child’s risk of relapse.

**Understanding Multicystic Dysplastic Kidney (MCDK) in Children**

Multicystic Dysplastic Kidney (MCDK) is a congenital condition where one kidney is replaced by multiple fluid-filled cysts and does not function. It typically develops during fetal life and is often diagnosed during routine antenatal ultrasound. The affected kidney does not filter blood or produce urine, but thankfully, the other kidney usually develops normally and takes over the entire kidney function.

Childhood Nephrotic Syndrome – What Parents Should Know

Nephrotic Syndrome is one of the most common kidney conditions seen in children. As a parent, it’s natural to feel concerned when you hear this diagnosis. Understanding the condition can help you support your child better.

What is Nephrotic Syndrome?
Nephrotic Syndrome happens when the kidneys leak too much protein into the urine. Normally, protein stays in the blood and helps maintain the right balance of fluids in the body. When too much protein is lost, it leads to swelling (especially around the eyes, legs, and abdomen), weight gain, and sometimes infections.

What are the signs to watch for?

Swelling, especially around the eyes (most noticeable in the morning)

Swelling of the feet, ankles, or abdomen

Foamy or frothy urine

Weight gain from fluid buildup

Tiredness and irritability

What causes it?
In most children, the exact cause isn’t known. The most common type is called Minimal Change Disease, which usually responds well to treatment. Other types are less common and may require specialized care.

Is it treatable?
Yes! The mainstay of treatment is steroids. Most children respond very well and can lead normal, active lives. However, relapses can happen, especially with infections or vaccinations.

What can parents do?

Follow the treatment plan: Give medicines exactly as prescribed.

Monitor for relapses: Watch for signs of swelling or changes in urine.

Prevent infections: Good hand hygiene, timely vaccinations, and keeping your child away from sick contacts are important.

Keep regular follow-ups: Your doctor may monitor urine protein levels and adjust medications.

Maintain a healthy diet: A low-salt diet is often recommended during swelling episodes.

When should you seek immediate help?

If your child has breathing difficulty due to severe swelling

If there is a fever, as infections need urgent treatment

If there is reduced urine output

Remember:
Childhood Nephrotic Syndrome can seem overwhelming, but with the right care and support, most children do very well. Early detection, regular follow-up, and good communication with your child’s doctor are key!
