Genetic issues affecting conception:
Genetic problems generally relate to chromosomal abnormalities and single gene issues. These often cause embryo implantation failure and pregnancy loss, as well as possibly resulting in birth defects when a child is born with a chromosomal or single gene defect.

Chromosomal disorders can relate to one or more of an embryo’s chromosome pairs containing either one too many or one too few chromosomes. When the chromosome is not paired correctly, it is called aneuploidy. A chromosome may also have an abnormal structure. Both kinds of chromosome problems can be present in the female egg and/or the male s***m.

Single gene defects result from the embryo inheriting a genetic flaw from one or both parents. The gene could be one that results in cystic fibrosis or muscular dystrophy in birth. These single-gene defects can also cause embryo implantation failure or pregnancy loss.

Genetic issues often can be identified through preimplantation genetic screening, two types of embryo evaluation during IVF that can identify either form of genetic issues.

Unexplained infertility
Unexplained infertility is the diagnosis that fertility specialists give an individual or couple when fertility tests can not confirm a definitive cause for infertility. Unexplained infertility does not mean that there is not a cause for infertility; it simply means that the tests and examinations are not able to detect the reason an individual or couple is struggling to conceive.

Unexplained infertility is the diagnosis in about 10-15% of infertility cases.

When is unexplained infertility the diagnosis?
Individuals and couples will receive an unexplained infertility diagnosis when the following conditions exist.

The woman is ovulating regularly and has an appropriate ovarian reserve (antral follicle count).
The female or female partner has at least one open fallopian tube (evaluated using a hysterosalpingogram).
The male or male partner’s semen analysis is normal (including s***m count, shape and movement).
Exams of the individual or of both partners did not result in the diagnosis of conditions or structural abnormalities that can cause infertility, such as uterine fibroids in women or varicoceles in men.
Unexplained infertility treatment
The inability to pin down a diagnosis with current fertility testing technology does not mean that a couple cannot get pregnant. In some cases, simple lifestyle changes or weight loss can improve the chances of pregnancy.

Fertility treatments such as fertility medications combined with IUI (intrauterine insemination) or in vitro fertilization (IVF) can dramatically improve the chances of pregnancy for those struggling with unexplained infertility.

Secondary infertility is the inability to become pregnant or to carry a baby to term after previously giving birth to a baby. Secondary infertility shares many of the same causes of primary infertility.

Secondary infertility might be caused by:

Impaired s***m production, function or delivery in men
Fallopian tube damage, ovulation disorders, endometriosis and uterine conditions in women
Complications related to prior pregnancy or surgery
Risk factor changes for you or your partner, such as age, weight and use of certain medications
If you're younger than 35 and have frequent, unprotected s*x but don't become pregnant after a year, talk to your health care provider. If you're 35 or older — or you have other infertility risk factors such as infrequent periods or endometriosis — earlier evaluation is recommended.

Depending on the circumstances, both you and your partner might need medical evaluations. Your doctor can help determine whether there's an issue that requires a specialist or treatment at a fertility clinic.

Ovulation is the release of an egg from one of the ovaries. It often happens about midway through the menstrual cycle, although the exact timing may vary.

In preparation for ovulation, the lining of the uterus, or endometrium, thickens. The pituitary gland in the brain stimulates one of the ovaries to release an egg. The wall of the ovarian follicle ruptures at the surface of the o***y. The egg is released.

Finger-like structures called fimbriae sweep the egg into the neighboring fallopian tube. The egg travels through the fallopian tube, propelled in part by contractions in the fallopian tube walls. Here in the fallopian tube, the egg may be fertilized by a s***m.

If the egg is fertilized, the egg and s***m unite to form a one-celled entity called a zygote. As the zygote travels down the fallopian tube toward the uterus, it begins dividing rapidly to form a cluster of cells called a blastocyst, which resembles a tiny raspberry. When the blastocyst reaches the uterus, it implants in the lining of the uterus and pregnancy begins.

If the egg isn't fertilized, it's simply reabsorbed by the body — perhaps before it even reaches the uterus. About two weeks later, the lining of the uterus sheds through the va**na. This is known as menstruation.

However, there are several things that women can do to decrease the possibility of infertility:

Primary ovarian insufficiency: — also called premature ovarian failure — occurs when the ovaries stop functioning normally before age 40. When this happens, your ovaries don't produce normal amounts of the hormone estrogen or release eggs regularly. This condition often leads to infertility.

Primary ovarian insufficiency is sometimes confused with premature menopause, but these conditions aren't the same. Women with primary ovarian insufficiency can have irregular or occasional periods for years and might even get pregnant. But women with premature menopause stop having periods and can't become pregnant.

Restoring estrogen levels in women with primary ovarian insufficiency helps prevent some complications that occur as a result of low estrogen, such as osteoporosis
Symptoms:
Signs and symptoms of primary ovarian insufficiency are similar to those of menopause or estrogen deficiency. They include:

Ovulation disorders
Ovulation disorders, meaning you ovulate infrequently or not at all, account for infertility in about 1 in 4 infertile couples. Problems with the regulation of reproductive hormones by the hypothalamus or the pituitary gland, or problems in the o***y, can cause ovulation disorders.

Polycystic o***y syndrome (PCOS).
PCOS causes a hormone imbalance, which affects ovulation. PCOS is associated with insulin resistance and obesity, abnormal hair growth on the face or body, and acne. It's the most common cause of female infertility.
Hypothalamic dysfunction.
Two hormones produced by the pituitary gland are responsible for stimulating ovulation each month — (FSH) and luteinizing hormone (LH). Excess physical or emotional stress, a very high or very low body weight, or a recent substantial weight gain or loss can disrupt production of these hormones and affect ovulation. Irregular or absent periods are the most common signs.
Premature ovarian failure.
Also called primary ovarian insufficiency, this disorder is usually caused by an autoimmune response or by premature loss of eggs from your o***y (possibly from genetics or chemotherapy). The o***y no longer produces eggs, and it lowers estrogen production in women under the age of 40.
Too much prolactin.
The pituitary gland may cause excess production of prolactin (hyperprolactinemia), which reduces estrogen production and may cause infertility. Usually related to a pituitary gland problem, this can also be caused by medications you're taking for another disease.

Damage to fallopian tubes (tubal infertility)
Damaged or blocked fallopian tubes keep s***m from getting to the egg or block the passage of the fertilized egg into the uterus. Causes of fallopian tube damage or blockage can include:

Pelvic inflammatory disease, an infection of the uterus and fallopian tubes due to chlamydia, gonorrhea or other s*xually transmitted infections.
Previous surgery in the abdomen or pelvis, including surgery for ectopic pregnancy, in which a fertilized egg implants and develops in a fallopian tube instead of the uterus.
Pelvic tuberculosis, a major cause of tubal infertility worldwide, although uncommon in the United States.
Endometriosis
Endometriosis occurs when tissue that normally grows in the uterus implants and grows in other locations. This extra tissue growth — and the surgical removal of it — can cause scarring, which may block fallopian tubes and keep an egg and s***m from uniting.

Endometriosis can also affect the lining of the uterus, disrupting implantation of the fertilized egg. The condition also seems to affect fertility in less-direct ways, such as damage to the s***m or egg.

Uterine or cervical causes
Several uterine or cervical causes can impact fertility by interfering with implantation or increasing the likelihood of a miscarriage:

Benign polyps or tumors (fibroids or myomas) are common in the uterus. Some can block fallopian tubes or interfere with implantation, affecting fertility. However, many women who have fibroids or polyps do become pregnant.
Endometriosis scarring or inflammation within the uterus can disrupt implantation.
Uterine abnormalities present from birth, such as an abnormally shaped uterus, can cause problems becoming or remaining pregnant.
Cervical stenosis, a narrowing of the cervix, can be caused by an inherited malformation or damage to the cervix.
Sometimes the cervix can't produce the best type of mucus to allow the s***m to travel through the cervix into the uterus.

Endometriosis:
Endometriosis is a gynecologic condition characterized by endometrial tissue from the inner lining of the uterus growing outside of the uterine cavity. Typical symptoms of endometriosis are painful menstrual periods, va**nal bleeding between periods, and infertility; however, some women with infertility associated with endometriosis have no symptoms. Although the exact mechanism of how endometriosis causes infertility remains poorly understood, the strong association between endometriosis and unexplained infertility exists. Many studies have shown that treatment of moderate and severe endometriosis can improve fertility in women attempting to conceive on their own or with fertility treatments. For women with endometriosis, surgery can be done to remove, or lessen, the amount of abnormal tissue in the pelvis. This can be achieved through:

Laparoscopy: Laparoscopy can be used to diagnose and treat endometriosis in a single procedure. The surgeon makes a tiny incision below the navel and places a small telescope into the abdominal cavity, allowing the doctor to visualize any scarring or abnormal tissue with a fiber optic camera. Additional small incisions can be made to introduce additional instruments needed to perform surgery.

Mini-laparotomy: The surgeon makes an incision in the abdominal wall to reach the affected areas. Mini-laparotomy is performed through a small incision located just above the p***c bone. The surgeon explores the pelvis and corrects the problem.

Laparotomy: In a laparotomy, an incision is made in the lower abdomen which allows the surgeon direct access to all structures of the pelvis. What causes infertility in men?
Infertility in men can be caused by different factors and is typically evaluated by a semen analysis. When a semen analysis is performed, the number of s***m (concentration), motility (movement), and morphology (shape) are assessed by a specialist. A slightly abnormal semen analysis does not mean that a man is necessarily infertile. Instead, a semen analysis helps determine if and how male factors are contributing to infertility.

Disruption of testicular or ejaculatory function
Varicoceles, a condition in which the veins on a man’s testicles are large and cause them to overheat. The heat may affect the number or shape of the s***m.
Trauma to the te**es may affect s***m production and result in lower number of s***m.
Unhealthy habits such as heavy alcohol use, smoking, anabolic steroid use, and illicit drug use.
Use of certain medications and supplements.
Cancer treatment involving the use of certain types of chemotherapy, radiation, or surgery to remove one or both testicles
Medical conditions such as diabetes, cystic fibrosis, certain types of autoimmune disorders, and certain types of infections may cause testicular failure.
Hormonal disorders:
Improper function of the hypothalamus or pituitary glands. The hypothalamus and pituitary glands in the brain produce hormones that maintain normal testicular function. Production of too much prolactin, a hormone made by the pituitary gland (often due to the presence of a benign pituitary gland tumor), or other conditions that damage or impair the function of the hypothalamus or the pituitary gland may result in low or no s***m production.
These conditions may include benign and malignant (cancerous) pituitary tumors, congenital adrenal hyperplasia, exposure to too much estrogen, exposure to too much testosterone, Cushing’s syndrome, and chronic use of medications called glucocorticoids.

Consider that the average woman of reproductive age only has a 20 percent chance of getting pregnant in any month of trying. And if conditions are hampering male and/or female reproductive systems, the chances are much worse. Infertility – defined as not conceiving after 12 months of having s*x on a regular basis with no contraception – affects as many as 1 in 8 couples.

Causes of infertility can range from not timing in*******se properly to being overweight to structural problems with the reproductive organs. Reproductive Science Center of New Jersey’s goal is to find out what factors are causing infertility in a woman or couple and develop a personalized treatment plan to help them achieve pregnancy.

The primary causes of failure to achieve pregnancy fall into three categories: medical issues, lifestyle factors and genetics.
