🧠 A complex, giant ruptured aneurysm of the right Middle Cerebral Artery (MCA) — managed microsurgically.

The pre-operative angiogram showed a large, multilobulated aneurysm arising from the M1–M2 bifurcation with compromised distal flow.
The post-operative angiogram demonstrates complete clipping and preservation of distal branches, marking a successful surgical outcome.

🧠 Cerebellopontine Angle Tumor – Completely Excised with Facial Nerve Preservation

The left image shows a contrast-enhancing tumor at the left cerebellopontine (CP) angle, compressing the brainstem and cerebellum — classic for a vestibular schwannoma, also known as an acoustic neuroma. These tumors arise from the vestibular division of the 8th cranial nerve and often present with hearing loss, imbalance, or facial numbness.

🎯 Surgical management was done via a retrosigmoid suboccipital craniotomy using microsurgical dissection under high magnification.

⚡️ Intraoperative neuromonitoring (IONM) was employed throughout the procedure to continuously monitor facial nerve function, enabling safe dissection and preventing nerve damage.

📈 The post-operative MRI (right) shows complete tumor excision with no residual lesion and beautifully decompressed brainstem.

👇 And most importantly, the patient is doing well — fully alert, facial nerve function completely preserved, no deficits, and recovering smoothly.

Case Update

Today the First CAROTID AI OCT GUIDED CASE performed from Velammal Hospital Madurai in which we have Performed the assessment of post carotid artery stenting today.

Patient: 77-year-old male with recent TIA
Diagnosis: 85% stenosis of proximal ICA
Procedure: DSA with carotid stenting ( 5*24)

Post-stenting:

Used Optical Coherence Tomography (OCT) to assess stent apposition, plaque morphology, and vessel wall integrity.

About OCT:
OCT is a high-resolution intravascular imaging technique using near-infrared light, offering ~10–20 micron resolution. It allows precise visualization of stent expansion, apposition, and detection of subtle issues like edge dissection or thrombus, often missed by angiography.

OCT MAX( Malapposition | Apposition | Expansion) and Pathophysiological Findings of this case,

OCT confirmed Distal malapposition | Mild under expansion with 15.69 Area | Prox edge mild Tissue prolapse | No Dissections @ Both the edges.

Proximal EEL - 5.5
Proximal Lumen- 4.55

Plaque characteristics - Tight segment has Fibrofatty plaque with superficial thin sheet of calcium.

Today, we mourn the passing of a legend—Prof. Gazi Yaşargil, the father of modern microneurosurgery. To those of us who had the privilege of knowing him, he was more than a pioneer—he was a mentor, an inspiration, and a symbol of surgical excellence.

His hands transformed neurosurgery. His vision reshaped our understanding of what was possible. Through Zurich, he touched the lives of thousands of patients, and mentored countless surgeons who carry his legacy into every operating room.

To know him personally was to witness humility wrapped in genius. He taught not just techniques, but philosophy—the elegance of precision, the importance of discipline, and the quiet power of compassion.
