3️⃣ आंत (Intestine) में मौजूद एंज़ाइम बिलिरुबिन को वापस बढ़ा देते हैं

नवजात की आंत में β-glucuronidase नाम का एंज़ाइम काफी सक्रिय होता है।
यह एंज़ाइम बिलिरुबिन को दोबारा “इनडायरेक्ट” रूप में बदल देता है।

इससे लीवर का काम और बढ़ जाता है

और इनडायरेक्ट बिलिरुबिन दोबारा खून में वापस चला जाता है

बड़े होने पर यह एंज़ाइम कम सक्रिय होता है, इसलिए बड़ों में बिलिरुबिन की री-सर्कुलेशन कम होती है।

4️⃣ बच्चे की आंत और लीवर के बीच का कनेक्शन (Enterohepatic circulation) ज़्यादा मजबूत

नवजात में ख़ास बात यह होती है कि
आंत से खून में बिलिरुबिन के वापस जाने की प्रक्रिया बहुत तेज़ होती है,
इसे कहते हैं — Enterohepatic circulation.

बच्चा बार-बार दूध नहीं पीता

पेट खाली रहता है

स्टूल (पाखाना) भी कम होता है

इससे बिलिरुबिन शरीर से बाहर नहीं निकल पाता और शरीर में जमा होता है।

5️⃣ ब्रेस्टफीडिंग के कारण भी हल्का बिलिरुबिन बढ़ सकता है

दो कारण होते हैं:
(A) Breastfeeding jaundice – दूध कम पीने पर
अगर बच्चा शुरुआत में ठीक से दूध नहीं पी पाता, तो डिहाइड्रेशन हो जाता है और बिलिरुबिन बढ़ जाता है।

(B) Breast milk jaundice – दूध के प्राकृतिक तत्वों से
कुछ बच्चों में माँ के दूध में मौजूद पदार्थ बिलिरुबिन के टूटने की गति को धीमा कर देते हैं।

यह स्थिति हानिकारक नहीं होती और धीरे-धीरे स्वतः ठीक हो जाती है।

6️⃣ बड़ों में ऐसा क्यों नहीं होता?

बड़ों में इनडायरेक्ट बिलिरुबिन कम रहता है क्योंकि:

लीवर का एंज़ाइम सिस्टम पूरी तरह विकसित और मजबूत होता है

RBC टूटने की रफ्तार सामान्य होती है

आंत में बिलिरुबिन को वापस खून में भेजने वाले एंज़ाइम कम सक्रिय होते हैं

एंटरोहेपेटिक सर्कुलेशन स्थिर रहता है

शरीर लगातार बिलिरुबिन को बाहर निकालता रहता है

इसलिए बड़ों में इनडायरेक्ट बिलिरुबिन केवल तब बढ़ता है जब कोई बीमारी हो—जैसे hemolysis, liver disorder, दवा का असर आदि।
