After working with over 50 patients, I’ve realized that the best results don't come from the most complex exercises, but from the most practical ones.
​If you want to move the needle for your PD patients, you have to stop treating them like they’re "fragile" and start treating them like athletes with a dopamine deficit.

​The "Cheat Sheet" for your next session:
✅ Intensity: Hit that 80% Max HR.
✅ Timing: Use the 45-60 min "Post-Meds" window.
✅ Amplitude: Exaggerate everything.
✅ Dual-Task: Add cognitive load before discharge.

​Research (APTA 2022) confirms: High-intensity resistance and gait training are the gold standards. Let’s stop "stretching" and start "loading."

1. Fugl Meyer assessment: This is an impairmentbased test with items organized by sequential recovery
stages. A three-point ordinal scale is used to measure
impairments of volitional movement with grades ranging from 0 (item cannot be performed) to 2 (item can be fully performed).Subtests exist for UE
function, LE function, balance, sensation, ROM, and
pain. he cumulative test score for all components is 226 with availability of specific subtest scores (e.g., UE maximum score is 66, LE score 34; balance score 14).

2. STREAM: The STREAM is a clinical measure of voluntary movements and basic mobility following stoke. It consists of 30 items (test movements) distributed equally among three subscales: upper-limb movements, lowerlimb movements, and basic mobility items. Voluntary movement items explore out-of-synergy control and are scored using a 3-point scale. The maximum STREAM score is 70 with each
limb subscore worth 20 points and the functional mobility subscore worth 30 points.

3. Stroke impact scale: The SIS is a self-report measure developed to assess function and quality of life after stroke. It includes 59 items
organized into 8 subgroups: strength, memory and
thinking, emotions, communication, ADL, mobility,
hand function, and participation. he final item asks the
person to rate perceived recovery on a scale of 0 to 100 with 100 representing full recovery and 0 representing no recovery.

Research shows that structured forward trunk-oriented exercises especially those emphasizing anterior weight shift, trunk stabilization, and anticipatory postural control appear to improve trunk posture and aspects of balance that underlie retropulsion. These may help PD patients more effectively use forward momentum and corrective stepping reactions.

Given the early and severe postural instability in PSP, comprehensive exercise programs especially those enhancing trunk and postural strategies contribute to better balance scores and may reduce retropulsive tendencies. However, direct high-quality evidence on forward trunk-specific methods in PSP is still emerging.

Uses of this exercise:
➡️ Can enhance the ability to move CoM forward over the base of support.
➡️ Improves anticipatory postural adjustments.
➡️ Reinforce motor patterns that counter backward falls.
