Osteonecrosis of hip also called as AVN has become an epidemic post Covid. Young patients with painful hip unilateral or bilateral are commonly seen during the last four years all over the world. There are so many procedures both non-operative and operative to treat the lesion. An intertrochanteric osteotomy is an affective method to remove the dead bone, graft it and then rotate the head of femur to offload the lesion. It brings the healthy bone under the weight bearing dome and ensures healing of the lesion. The patient in question suffered from grade 3 AVN rt hip can walk for 10 kilometres after one year of surgery. The subchondral fracture with collapse is marked with red Arrows. The healing of the lesion marked with black arrows can be appreciated.
