Perforated empyema GB with early gangrenous changes with Mirizzi ( cholecystocholedochal fistula ie Gallbladder to CBD connection abnormal ) with stone partially slipped into CBD . Laparoscopic management.

1st picture shows the white Pus after majority is washed off after dissecting the area which was initially invisible and frozen into a phlegmon . Pus trickling from perforation site

2nd Picture shows, CBD flushing after stone extraction through fistula site before the site was excised and closed .

Advanced Cirrhosis ie CLD with Portal Hypertension and bad varices with liver nodule and Mirizzi syndrome acute Gallbladder stone disease ie Acute Cholecystitis. These are deadly combinations where even open surgeries are too challenging as liver can behave very unpredictably and go into Liver failure. They tend to bleed heavily during operation from abnormal high pressure blood vessels ( varices and collaterals ) and also from Liver itself as fibrotic liver doesnt collapse to allow easy clotting of bleeding veins and also these abnormal veins are very fragile and thin walled and has very high pressure .

Laparoscopic Hernia composite mesh Looks like this when Clean bloodless surgery is done .

These mesh are different from ones used on the Outside of abdomen or in groin because composite ones has inner Prolene ( usual in other mesh ) and outer ie one facing inside of abdominal peritoneal cavity ie intestines etc is made of absorbable innocuous non adhesive material which will not cause intestinal adhesions and hence mesh erosion and gut perforations ( although more in theory than in practical ) ( Because we have re operated for Recurrent ventral / incisional hernia and had seen composite mesh with lot of adhesions although less and also on the contrary , a prolene mesh , although rare , but used by previous surgical team having adhesions but never caused any gut perforation or erosion )

Although as Thumb rule , Composite ( and that is why they are Much more expensive meshes ) are used for intra peritoneal mesh placements
