What is ISTHMOCELE?

ISTHMOCELE occurs when a small pocket or pouch forms in this isthmus part of uterus, usually after a woman has had a cesarean section or other uterine surgeries.

Signs and Symptoms:

- Lower Abdominal Pain, especially during menstruation.
- Abnormal menstrual Bleeding or post menstrual spotting
- Chronic Pelvic pain (A feeling of pressure or discomfort in pelvis )
- Infertility (one of the very common symptom after previous caesarean section)

Complications

- Infertility
- placenta accrete or praevia
- scar dehiscence
- uterine rupture
- cesarean scar ectopic pregnancy

Transvaginal ultrasound and saline infusion sonohysterography are effective tools to diagnose isthmocele.

Management is decided on the basis of size of the defect, the presence of symptoms or secondary infertility.

Surgical management includes laparotomy, minimally invasive approaches (hysteroscopy and laparoscopy) and transvaginal approach.

Early detection and appropriate management can significantly improve the quality of life for individuals dealing with ISTHMOCELE. Regular check-ups and open communication with your doctor are key to ensuring optimal reproductive health.

Isthmocele is also known as Caesarean scar defect or Niche.

Symptoms of isthmocele- secondary infertility, abnormal uterine bleeding, post menstrual spotting, chronic pelvic pain.

Management includes medical and surgical management.

Surgical management- different options are there- hysteroscopy, laparoscopy, vaginal, laparotomy.

Laproscopy dermoid cystectomy

Presenting a case of 35 yr old female with complain of lower pain abdomen and her USG finding suggestive of right dermoid cyst of 6x6 size.

Pt was taken up for laproscopy dermoid cystectomy.

Ovarian dermoid cysts or mature cystic teratomas (MCT), are the most common benign ovarian tumor in adults and adolescents.

Symptoms include pain, pressure or a feeling of fullness in your abdomen and rarely it causes emergency when torsion or rupture occurs.

Not all ovarian dermoid cysts need to be removed. Removal of cyst is needed if it’s large (approximately greater than 5 cms in diameter), causing symptoms (pain) or increased possibility of malignancy.

Advantages of laproscopy dermoid cystectomy -
1. Laparoscopy is day care procedure and pt can be discharged on same day .
2. It provides panoramic view of abdominal cavity and other causes of pain can also be treated, in index case there was an endometriotic nodule on anterior abdominal wall which was excised as well.
3. Cosmetically better scar compared to open surgery.

Hysteroscopic polypectomy-

Presenting a case of 55 year old female with complaint of postmenopausal bleeding with Ultrasound finding suggestive of endometrial polyp of 1.5 cm size.

Patient was taken up for hysteroscopic polypectomy and endometrial biopsy.

Advantage of hysteroscopic polypectomy-
1. It's a day care procedure and patient can be discharged the same day.
3. It ensures complete removal of polyp under vision.
4. It is safer procedure compared to blind D&C.
6. It allows panoramic view of inside of uterus thus increasing accuracy and precision.
