10+2 Certificate, (Self attested) if any.
Category of Registration:
Medical Assistant/Pharmacist/Compounders Diploma/ D.Pharmacy / B.Pharmacy Certificate (self-attested), whichever is applicable, in which she/he was previously registered in the repealed J&K Pharmacy Act, Samvat, 2011.
Pharmacist Registration copy and Last Renewal thereof (self-attested).
One Character Certificate certified from concerned SHO/ Sarpanch/ Corporator / Ist Class Magistrate.
Aadhar CardCertificate/ Number.
