EARLOBE KELOID
Ear piercing is easily the most common reason in predisposed individuals
- Various treatment options are available including intralesional injections, cryotherapy, radiotherapy, laser and surgery
- Inview of high chance of recurrence, surgery is usually not an option of keloid treatment except earlobe keloids which has low risk of recurrence
- Adjunctive therapy with intralesional injection post removal also helps in prevention of recurrence of earlobe keloids

Cracked heels or Heel fissures
* Deficiency of sweating or decreased hydration of skin leads to splits or cracks on heels. It becomes deeper and thicker with increased pressure which results in severe pain and bleeding along with increased risk of infection.
* If not treated, it could act as a gateway for bacterial and fungal infection which might even lead to fatal complications in individuals with diabetes, vascular problems, etc

Risk factors
* Any systemic condition that can cause dry skin, e.g., hypothyroidism or diabetes or vascular disease or neuropathy
* Atopic dermatitis, juvenile plantar dermatosis, psoriasis, and palmoplantar keratoderma (hereditary or acquired)
Prevention
* Prevention is the best treatment for heel fissures
* Mild exfoliator and pumice stone to remove the unwanted callus
* Regular application of moisturizer even after resolution of fissures is important to prevent the recurrence
* Apply topical anti-bacterial creams or ointments at an early stage to keep the infection under check
* The risk of recurrence can be significantly reduced by use of in-shoe devices such as heel cups, insoles to redistribute bodyweight and provide more support for the heel.
What emollient to be used?
* Most commonly used product is 10% urea cream
* Combination of 10% urea + 5% lactic acid cream can also be considered.
* Product with higher urea content for people with severe fissures
* 6% Salicylic acid cream can be used as a keratolytic which reduces hyperkeratosis and fissures
* Alpha hydroxy acid-based products (glycolic, citric and lactic acids)
* Saccharide isomerate (pentavitin) - long lasting moisturisation
* Urea products and SA products can cause discomfort and stinging when applied to broken skin if the dermal layer is exposed.
* Efficacy is improved when applied under occlusion
* Preferably should be applied at night to reduce the risk of falls
Other treatment options
* Treating the fissures with a liquid, gel or spray bandage or tissue glue, e.g. cyanoacrylate liquid skin protectant. 🇦🇪 🇦🇪

ACANTHOSIS NIGRICANS (AN)

Thickened velvety like hyperpigmentation commonly noted on sides and back of neck, underarms and groins.
Similar lesions on face, elbows and knuckles are also not uncommon
Mucosal or perioral involvement, palmar ridges involvement with progressive unexplained weight loss and rapid development of AN could be warning signs of cancer.

AN may be a sign of developing metabolic syndrome or concomitant existence of diabetes, hypertension and central obesity.

We are increasingly noticing the problem in children which parallels the incidence of childhood obesity as well.

Tests

Fasting serum insulin and glucose levels, and a fasting lipid profile should be checked in patients with syndromic, obesity-associated and benign AN.
If cancer is suspected

Treatment
No gold standard treatment available till date
Lifestyle changes - Low calorie diet, weight reduction, physical exercises

Topical retinoids are considered one of the first-line treatment options for AN

Combination of 0.05% tretinoin cream and 12% ammonium lactate cream - used in obesity-related AN based on some anecdoctal case reports

Calcipotriene, a vitamin D analog, also being tried as an alternative treatment option

Superficial chemical peels (Trichloroacetic acid in particular) are also a safe and effective treatment option in hands of well trained physician

Oral retinoids, conventional insulin-sensitizing agents such as metformin, combination of metformin with thiazolidiones are also used in the treatment of AN under strict guidance of treating physician.

Almost half of the patients with facial acne suffers from truncal acne (pimples on chest wall, shoulders, arms and back) with predominance among males.
More than 75% respondents in a survey desired for the treatment of truncal acne
Major challenge faced in the treatment of truncal acne is the extensive body surface area involvement
Combination approach is required to reach an optimal therapeutic benefit
Scarring is a common sequelae similar to facial acne.

Treatment options
Topical benzoyl per oxide wash (bleaching property on clothes is warranted), clindamycin, topical retinoids.
Systemic options include oral antibiotics, isotretinoin and hormonal therapies (combined oral contraceptives) but are strictly to be used under physician supervision.
TRIFAROTENE 0.005%
First topical treatment studied and proven safe to treat facial and truncal acne approved by USFDA in October, 2019. It’s a new retinoid to be approved for the treatment of acne in more than 20 yrs. (last approval was for tazarotene in 1997).

My friend tried hands on avocado ice cream for the first time and surprisingly it turned out to be a delicious delight. That was really an awesome dessert.

Avocado (Persea americana) originated from south central Mexico is a large berry containing a single large seed. It is called by the name butter fruit in many parts of India owing to dense and creamy nature of the fruit.
